About this file

This document contains a past performance questionnaire template to be completed for a federal contract opportunity. The past performance questionnaire requests evaluation of a contractor's quality of service, timeliness, business relations, management of key personnel, cost management, and compliance with requirements on an identified contract. It asks the evaluator to rate performance across these elements as low, moderate or high risk, or unknown. It also includes questions regarding notices of noncompliance, option exercises, contract discrepancies, terminations and cure notices. Overall ratings of contractor performance and willingness to hire the contractor again are also requested. The completed questionnaire will be used to evaluate offerors for solicitation number 1605C1-21-R-00012, whereby the Department of Labor seeks form 5500 research subscription and data analysis services in accordance with the attached performance work statement. Responses are due by August 23, 2021 to Johnson.Daveana@dol.gov.
