About this file

This is a Conflict of Interest Affidavit form for the Maryland School for the Deaf's procurement of 39 Cisco Access Points and associated mounts and brackets (PORFP R99P6601092). The affidavit serves as a required certification document for bidders participating in this state contract opportunity. The form establishes the legal framework through which prospective vendors must disclose any potential conflicts of interest that could compromise their ability to perform the contract objectively or create undue influence in the procurement process. Bidders are required to complete and submit this affidavit as part of their bid package to ensure compliance with Maryland's procurement regulations and ethical standards.

The affidavit requires vendors to declare any financial interests, business relationships, familial connections, or other circumstances that might present conflicts of interest relative to the Maryland School for the Deaf, its employees, or board members. This disclosure mechanism protects the integrity of the competitive bidding process and ensures that contract awards are made based on merit and compliance with procurement law rather than undisclosed relationships or improper influences. Failure to accurately complete this affidavit or failure to disclose known conflicts of interest may result in bid rejection, contract termination, or other remedial actions as determined by the state.
