About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing requirements for contractors entering into agreements with San Mateo County. The document serves as a binding assurance that contractors will comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations and guidelines. The assurance applies to all contracts awarded after the document's issuance date and remains binding on the contractor, its successors, transferees, and assignees. Contractors must certify their compliance status by completing the form, which requires authorization from a person with signatory authority on behalf of the organization.
The document establishes two compliance pathways based on contractor size: contractors employing fewer than 15 persons have a reduced compliance burden with an exception allowing referral of disabled persons to other accessible providers if facility alterations would constitute a significant burden, while contractors employing 15 or more persons must designate a specific Section 504 coordinator responsible for ensuring organizational compliance with DHHS accessibility regulations. Contractors are required to provide their name, address, designated 504 coordinator name (if applicable), signature, title of authorized official, and certification date on the form to demonstrate understanding and agreement with these obligations.
