About this file
This is a Section 504 Compliance Assurance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors seeking contracts with San Mateo County. The document requires all contractors to certify their compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations. Contractors must designate whether they employ fewer than 15 persons or 15 or more persons; those employing 15 or more employees must identify a Section 504 coordinator responsible for compliance efforts. The form requires the contractor's authorized official to provide their name, title, street address, city, state, and zip code, along with a signature and date certifying the accuracy of the information provided.
The assurance is binding on the contractor, its successors, transferees, and assignees, and contractors acknowledge that contracts will be extended in reliance upon the representations and agreements made in the document. The form includes an exception provision stating that DHHS regulations permit recipients with fewer than 15 employees to refer disabled persons to other accessible service providers if no method of compliance exists without making significant alterations to existing facilities. This compliance form represents a standard requirement for doing business with San Mateo County and serves to ensure non-discriminatory practices in service delivery to individuals with disabilities.
