About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013. The document establishes compliance requirements for contractors entering into agreements with San Mateo County regarding adherence to Section 504 of the Rehabilitation Act of 1973, as amended, and applicable Department of Health and Human Services (DHHS) regulations. The assurance applies to all contracts executed after the date of issuance and is binding on the contractor, its successors, transferees, and assignees. The document requires authorized officials to certify compliance with federal accessibility and non-discrimination requirements in consideration of obtaining county contracts.
The document distinguishes compliance obligations based on contractor size, with separate requirements for contractors employing fewer than 15 persons versus those employing 15 or more persons. Contractors with 15 or more employees must designate a Section 504 Coordinator responsible for coordinating compliance efforts with DHHS regulations. The form requires contractors to provide the name and contact information of the designated 504 Coordinator, if applicable, along with standard contractor identification information. A limited exception is provided under DHHS regulations allowing contractors with fewer than 15 employees to refer individuals with disabilities to alternative accessible service providers if facility modifications would constitute a significant alteration to existing facilities. The authorized official must certify the accuracy and completeness of submitted information.
