About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the Overpayment Resolution Services contract opportunity (RFP No. 66000-2526-225) for San Mateo Medical Center. The document requires contractors to certify compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations. Contractors must indicate whether they employ fewer than 15 persons or 15 or more persons; those employing 15 or more persons must designate a specific individual to coordinate compliance efforts with DHHS regulations. The assurance is binding on the contractor, its successors, transferees, and assignees, and represents a condition for contract award.
The document requires submission of contractor identification information including the name of the designated 504 compliance person (if applicable), contractor name, street address or P.O. Box, city, state, and zip code, along with an authorized official's signature, title, and date. The contractor must certify that all provided information is complete and correct. An exception clause notes that contractors with fewer than 15 employees may refer disabled persons seeking services to other accessible providers if facility modifications would constitute a significant alteration to existing facilities. This compliance assurance is a mandatory requirement for all proposers responding to the County of San Mateo's Overpayment Resolution Services solicitation.
