About this file
This document is an Assurance of Compliance with Section 504 of the Rehabilitation Act of 1973 issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the Copying Services for Legal Imaging Request contract opportunity with San Mateo Medical Center. The form requires contractors to certify their compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. Contractors must indicate their employee count: those employing fewer than 15 persons or those employing 15 or more persons must designate a Section 504 compliance coordinator. The assurance is binding on the contractor, its successors, transferees, and assignees, and must be signed by an authorized official.
The document includes an exception clause noting that recipients with fewer than 15 employees may refer disabled persons seeking services to other accessible providers if compliance would require significant alterations to existing facilities. No specific pricing terms, set-asides, incumbents, funding sources, or contract term information is contained within this compliance assurance form. This is a standard civil rights compliance requirement that all contractors must complete and certify as a condition of contract award.
