About this file

This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing compliance requirements for contractors entering into agreements with the county. The document requires all contractors to comply with Section 504 of the Rehabilitation Act of 1973, as amended, along with all applicable Department of Health and Human Services (DHHS) regulations and related guidelines. The assurance applies to contracts executed after the document's issuance date and is binding on the contractor, its successors, transferees, and assignees. Contractors must certify their compliance by completing the form with authorized official signatures and dates.

The compliance form differentiates between contractors based on employee count, with those employing fewer than 15 persons subject to one set of requirements and those employing 15 or more persons required to designate a Section 504 coordinator responsible for compliance efforts. The document includes an exception allowing contractors with fewer than 15 employees to refer individuals seeking services to other accessible providers if making facility alterations to achieve compliance would constitute a significant burden. Contractors must provide complete and accurate information regarding their organization name, address, designated compliance coordinator (if applicable), and authorized official certification.
