About this file

This document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, specifically related to Section 504 of the Rehabilitation Act of 1973. The form requires contractors to affirm their commitment to complying with federal regulations regarding non-discrimination and accessibility for individuals with disabilities. Contractors must indicate whether they employ fewer than or 15 or more persons, and if the latter, they must designate a specific person to coordinate efforts to comply with DHHS regulations.

The form serves as a legal assurance that the contractor will adhere to facility accessibility guidelines, with an exception noted that organizations with fewer than 15 employees may refer disabled individuals to alternative accessible service providers if making significant facility alterations is not feasible. The document is designed to be signed by an authorized official, certifying the completeness and accuracy of the information provided, and becomes binding on the contractor, its successors, transferees, and assignees. While this specific document is part of a broader procurement process for Freight Management Services at San Mateo Medical Center, the form itself is a standard compliance document focused on disability access and non-discrimination requirements.
