About this file
This is a Responsibility Inquiry form (Attachment F) from the State of Oregon's Department of Administrative Services for RFP #S-58100-00009792 regarding the Oregon Healthy Schools Program Evaluation. The form functions as a pre-award responsibility assessment tool that requires bidders to demonstrate their financial, material, equipment, facility, and personnel resources necessary to fulfill contractual obligations. The inquiry covers multiple evaluation criteria including bidder's experience with similar contracts completed within budget and on schedule over the past five years, criminal or civil charges within the last three years, contract terminations for default, outstanding judgments, and financial stability. Bidders must also confirm possession of all required licenses, insurance, and registrations to conduct business in Oregon, and if applicable, provide evidence of a current Pay Equity Certificate issued by the Department of Administrative Services.
The form requires bidders to provide detailed responses to standardized questions addressing financial distress, cash flow sufficiency, bankruptcy filings, and any lawsuits or creditor claims filed within the last three years. Failure to promptly provide requested information or clearly demonstrate responsibility may result in an Agency finding of non-responsibility and rejection of the bid. The form requires an authorized representative's signature certifying that all responses are complete, accurate, and not misleading. This responsibility inquiry is a mandatory component of the bid evaluation process and precedes contract award and execution, ensuring that selected contractors possess the requisite capability and integrity to perform state contracts.
