About this file
This document is an informational flyer issued by the South Carolina Department of Social Services (SCDSS) Office of Child Health and Well-Being, specifically DSS Form 37151, providing guidance to caregivers and DSS case managers regarding health care requirements for children in foster care. The flyer outlines mandatory well child visit and dental visit protocols designed to ensure comprehensive health assessments for foster children. Initial well child and dental visits must occur within 30 days of a child entering foster care, with an ideal timeframe of seven days. Following the initial visit, children must receive ongoing well child visits based on age-specific schedules aligned with American Academy of Pediatrics guidelines, and routine dental check-ups must be scheduled every six months per American Academy of Pediatric Dentistry guidelines. Well child visits must include a health history, complete physical examination, laboratory tests as needed, immunizations, vision and hearing screening, and developmental or behavioral screening. Additionally, children may receive oral evaluations and fluoride varnish applications during primary care visits, with children up to seven years old eligible for up to four fluoride varnish applications annually.
The flyer emphasizes the critical importance of documentation and communication between healthcare providers, foster caregivers, and SCDSS nurse care coordinators. After-visit summaries or clinical summaries from healthcare appointments must be submitted to childhealth@dss.sc.gov to ensure proper tracking of health care needs and treatment completion. These summaries should document visit reasons, administered immunizations, prescribed medications, appointment dates and times, and recommended follow-up appointments or tests. The document notes that Medicaid covers initial dental visits regardless of prior dental visit timing, and Select Health allows multiple well child visits annually. The SCDSS Office of Child Health and Well-Being provides support contact information via email at childhealth@dss.sc.gov or by telephone at (803) 898-7788 for providers and caregivers requiring assistance or clarification regarding health care benefits and requirements for foster children.

Well Child and Dental Visits
On-going Well Child Visits:
After the initial visit, all children must receive follow-up well child visits based on their age. This schedule is based on the American Academy of Pediatrics guidelines for children in Foster Care. These visits help the primary care physician provide proper referrals and follow up on the child’s health care needs.
The SCDSS Office of Child Health and Well-Being needs your help to implement new requirements to meet the health care needs for children in foster care.
A well child visit can occur at the same time as a sick visit but should also contain the following elements:
• Health history
• Complete physical exam
• Lab tests (as needed)
• Immunizations
• Vision and hearing screening
• Development/behavioral screening (as needed)
• Advice on how to keep your child healthy
During each well child visit, children in foster care should receive an oral evaluation and can receive fluoride varnish from their primary care doctor.
Ongoing Dental Check-Up Visits:
After the initial dental visit, all children must receive routine dental check-up visits every six months.
This schedule is based on the American Academy of Pediatric Dentistry guidelines for children up to 20 years of age.
TIP: Medicaid allows the initial dental visit to occur regardless of the last time the child saw a dentist. If the dental provider is not aware of this benefit, email us at: childhealth@dss.sc.gov
TIP: Select Health allows several Well Child Visits per year. If your provider is not aware of this benefit, email us at: childhealth@dss.sc.gov We can connect them to someone who can assist your provider.
TIP: Ask your child’s doctor about fluoride varnish. Children up to seven years old can get four applications of fluoride varnish every 12 months.
If your child does not have a dentist, please ask for a referral to a dental provider.

After Visit Summaries SCDSS Office of Child Health and Well-Being needs your help to make sure that nurse care coordinators at SCDSS are aware of children’s health care needs and can track treatment until those needs are completed.
We do that by receiving “After Visit Summaries” or “Clinical Summaries” that primary care providers give you at the appointment.
DSS nurse care coordinators will review those summaries and provide follow-up action items for the child’s team.
TIP: You can use your cell phone to take a picture of the after visit summary and email it to:
childhealth@dss.sc.gov
Well Child and Dental Visits
After visit summaries have important information such as the reason for the visit, immunizations administered during the visit, medications prescribed, the time and date of the next appointment, and other appointments and tests that need to be scheduled.
Send the after visit summary to: childhealth@dss.sc.gov. Please do not send the sheet that you take to check out that contains diagnosis codes because it does not have information that we need about follow-up needed for your child’s care.
During your monthly visit with your child’s foster care case manager, be sure to discuss any visits to the doctor and any follow-up needs that the doctor recommended. Also, give the case manager the after visit summary for their records so that it can be uploaded to the child’s file.
TIP: If your child’s dentist or primary care provider does not give you an after visit summary after the visit, ask for one. If one is not available, you can complete DSS Form 30260 Medical/Dental Encounter Form.
