About this file

This document is Attachment E containing three addenda to a Kentucky Request for Bids (RFB) for courier service to transport newborn screening (NBS) samples and related clinical and environmental specimens. The service involves daily pickups from 46 Kentucky birthing hospitals, 12 health departments, and one correctional facility, with delivery to the Kentucky Division of Laboratory Services (DLS) in Frankfort. Addendum No. 1 extended the vendors' question response deadline to Monday, July 13, 2026, and extended the bid closing date to Monday, July 20, 2026, at 1:30 PM EST. The projected contract start date is August 1, 2026. All newborn screening specimens must be delivered by 7:00 AM the following morning from pickup, with a 24/7 drop-off location available at DLS. Service is required seven days a week except for Thanksgiving Eve, Christmas Eve, and New Year's Eve, with health department and correctional facility pickups required daily after 4:00 PM and hospital pickups occurring Sunday through Friday after 5:00 PM. On-demand service for time-critical samples requiring delivery within 24 hours of notification is available during working hours (8:00 AM to 4:30 PM EST) and is predicted to occur less than five times annually.

Addendum No. 2 established specifications for NBS specimen storage (not exceeding 80 degrees Fahrenheit, preferably less than 30% humidity, and no direct sunlight) and clarified that specimens from health departments and correctional facilities must be maintained in an outer container with ice packs after pickup. Bidders should submit pricing as a flat monthly fee for Commodity Line 1 (regular pickups) and a flat rate for Commodity Line 2 (on-demand service), with the current contract value at $520,260 annually. Addendum No. 3 added comprehensive payment terms stating the Commonwealth will make payment within 30 working days of invoice receipt and acceptance of goods and services, with invoices requiring specific information including contract numbers, quantities, unit prices, and extended totals. A one-percent (1%) penalty accrues monthly on any undisputed amount owed after 30 working days following receipt of goods and invoice. Contractors must provide valid driver's licenses, maintain required insurance, and comply with HIPAA requirements; no current incumbent exists for this contract. Contractors are responsible for providing PPE, biohazard bags, coolers, and refrigerant, with daily courier logs required documenting pickup times, contact information, and envelope counts.
