About this file

This is a Vendor Pre-Operational Performance Report Card attachment for the Commonwealth of Kentucky Cabinet for Health and Family Services, Department for Medicaid Services (DMS) financial management services contract for Participant-Directed Services in 1915(c) Home and Community Based Services Medicaid waiver programs. The report card establishes twenty-one performance requirement categories that the selected vendor must meet during the pre-operational phase, including agenda preparation, change control and change management procedures, deliverable and artifact management, document management through DMS's Project Online environment, document review and standards, joint application development participation, comprehensive project management responsibilities, RACI matrix development, risk and issue management, stakeholder management, strategic planning facilitation, work breakdown structure development, schedule management, security compliance, staffing level maintenance at ninety percent or higher of the staffing plan, training approval and evaluation, and weekly status reporting. Key performance requirements include delivering the project work plan within twenty business days of contract execution, preparing the RACI matrix within thirty calendar days of contract signing, documenting risks and issues within two business days of identification, completing scheduled jobs within the scheduled timeframe or within two clock hours thereof, submitting training materials thirty calendar days prior to training delivery, and providing weekly status reports using DMS-approved templates with updated Gantt charts.

The performance report card serves as a pre-operational evaluation tool with each requirement marked as either Met or Not Met status. All requirements address vendor operational capabilities and deliverable quality standards, including maintaining business and technical writing standards that limit ambiguity and rework, ensuring adequate English proficiency to minimize grammar and clarity edits, and meeting content expectations as defined in the Deliverables Expectation Document. Additionally, the vendor must maintain federal and state privacy, confidentiality, and security requirements throughout operations and ensure appropriate experienced staffing is dedicated to the project in accordance with the approved Staffing Plan and project roles.
