About this file

The document is an Attachment D - Reference Laboratory Testing Experience Worksheet for a Department of Veterans Affairs (VA) solicitation for VISN22 Reference Lab Testing (Solicitation Number 36C26226R0001). The worksheet is designed for offerors to provide detailed information about their experience in reference laboratory testing from hospital settings, which will be used to evaluate proposals in STEP 2 of the evaluation process.

The worksheet requires offerors to comprehensively document specific reference contracts, including the legal entity name, contract dates, performed tasks, courier service areas, types of laboratory testing (such as genetic or drug testing), average monthly test volume, contractor status (prime or subcontractor), and contact information for contract verification. Offerors must complete a separate worksheet for each reference contract they wish to have considered, with the goal of demonstrating their expertise in providing reference laboratory testing services within a hospital environment for the VA's Veterans Integrated Service Network 22.
