About this file
This document is a Quality Assurance Surveillance Plan (QASP) for a federal contract to collect, transport, and dispose of regulated medical waste (RMW) for four Department of Veterans Affairs (VA) facilities. The QASP outlines the government's roles and responsibilities, performance standards and acceptable quality levels, incentives, and methods of quality assurance surveillance. It designates a Contracting Officer's Representative (COR) to administer the contract and monitor the contractor's performance. The QASP specifies five performance indicators, including timely waste collection, proper disposal, and submission of monthly and annual reports. The government will use a positive past performance and option period exercise as incentives, and will rate the contractor's performance as Excellent, Satisfactory, or Unsatisfactory based on meeting the stated standards. The QASP is a "living document" that the government may revise in coordination with the contractor.
The related federal contract opportunity is for the medical waste disposal services described in the QASP, with a solicitation number of 36C24625Q0192. The Department of Veterans Affairs, Veterans Health Administration, Veterans Integrated Service Network 6 is the procuring agency for this civilian contract.

During contract performance, the COR will periodically analyze whether the negotiated frequency of surveillance is appropriate for the work being performed.
