About this file
This is a Proposer Information Certificate form used by a state or local government entity to collect standardized information from vendors and contractors bidding on contract opportunities. The document serves as a certification vehicle for prospective proposers to provide organizational details, ownership structure, and business classification information required for bid evaluation and compliance purposes. The form requests comprehensive company background data including legal business name, principal place of business, years in operation, ownership structure (sole proprietorship, partnership, corporation, LLC, etc.), and organizational certifications. While the specific contract scope, deliverables, quantities, response deadlines, site visit requirements, and award timelines are not detailed within this particular form, it functions as a prerequisite documentation requirement that proposers must complete and submit alongside their formal bids and technical proposals for state and local contract opportunities.
The certificate form emphasizes compliance documentation and vendor qualification verification, typically collecting information relevant to set-aside programs, small business certifications, disadvantaged business enterprise (DBE) status, and other socioeconomic classifications that may affect contract award decisions or pricing considerations. Proposers are required to certify the accuracy of information provided under penalty of perjury, making this document a legally binding statement of company status and eligibility. The form does not establish pricing terms, contract amounts, or funding sources within its structure; rather, it serves as a qualifying document that establishes proposer eligibility and authenticity prior to substantive bid evaluation. Organizations must ensure all information is accurate and current, as misrepresentations may result in bid disqualification or contract termination.
