This document is a Past Performance Questionnaire template issued by the National Aeronautics and Space Administration (NASA) Strategy & Architecture Office for use in source selection for the Moon to Mars Architecture Studies initiative. The questionnaire is designed to be completed by organizations with firsthand experience with offerors to provide the Government with performance assessment data. Respondents are instructed to complete and submit the questionnaire electronically to hq-nextstep3appb@mail.nasa.gov, Attention: Maranda McCord, Contracting Officer, by an unspecified deadline (to be inserted by the Government). The completed questionnaire becomes part of the official Source Selection record under FAR 2.101 and 3.104.

The questionnaire comprises five sections covering: (1) Contract Identification, including contractor details, contract type, period of performance, dollar value, and description of work performed; (2) Customer or Agency Identification; (3) Respondent Identification, capturing the respondent's name, title, contact information, and role; (4) Performance Information, using a standardized five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) to evaluate Technical Performance (six questions), Program Management (seven questions), and Cost Performance (four questions), with supporting information required for each rating; and (5) General Comment, requesting an overall recommendation regarding contract award eligibility.
