Attachment C - Annual Required Affidavit for Bidders, Offerors, and Contractors

This document is an annual required affidavit form used by the Commonwealth of Kentucky for bidders, offerors, and contractors participating in state and local contract opportunities, including the Meade County College & Career Center Parts Washer Request. The affidavit serves as a compliance certification mechanism requiring bidders to attest to their eligibility and adherence to applicable state regulations and requirements. Bidders and contractors must complete and submit this form as part of their proposal package to demonstrate good faith compliance with Kentucky procurement regulations and contractor responsibility standards.

The affidavit form establishes the framework for contractor accountability and verification of bidder qualifications prior to award. Contractors executing this document certify their understanding of applicable laws, regulations, and procurement rules governing state and local contracts in Kentucky. The form functions as a mandatory submission requirement to validate that bidders meet established standards for participation in government contracts and possess the necessary authorization and legal standing to execute work on behalf of public entities within the commonwealth.
