STATEMENT OF NEED

1. BACKGROUND: The Malcom Randall VA Medical Center (VAMC), Gainesville, FL has need for one (1) Electric Powered Forklift (Brand Name or Equal) for its Supply Chain Management Service Department.

2. PURPOSE: This Statement of Need (SON) defines the requirements for the Electric Powered Forklift. Having the proper forklift functions and availability is vital to daily supply chain operations. The ability to strategically store, transport or relocate medical equipment and supplies internally at the demands of the VA mission is pivotal.

3. SCOPE: The Contractor shall provide and deliver the Electric Powered Forklift in accordance with the period of performance and place of delivery within the contract. Scope includes assembly and delivery of the product.

4. SALIENT CHARACTERISTICS: Referenced model: Toyota, 3-Wheel Sit-Down AC Electric Forklift (P/N: 8FBE20U. Product must meet the following specifications and requirements.

Physical/Salient Characteristics:

· Must have solid pneumatic tires

· Side shifter (hydraulically moves the fork carriage left and right simultaneously)

· Forks should be 42 ‘’ long

· 36 voltage electric battery powered. Desired input voltage of the charger - 36 VOLT

· Strobe and back-up alarm

Mast Specifications:

· Maximum Fork Height - 189.0”

· Overall Lowered Height - 83.5”

· Over Head Guard Height - 81.00”

· Free Lift - 35.6" with standard Load Backrest

Lifting Capacity:

· Base Model Capacity – 4,000 pounds at 24” load center

5. WARRANTY: Industry standard manufacturer warranty and service plans if available shall be included to cover all equipment. Government does not accept extended warranty, which requires advance payment.

6. DELIVERY SCHEDULE: All items to be delivered within the performance period as stated on the contract after receipt of purchase order and award.

7. SETUP AND TRANINIG: N/A

8. PLACE OF DELIVERY: Delivery hours are between 8:00AM to 4:00PM EST, Monday – Friday (Excluding National Holidays). Delivery will be to the following addresses:
