The safety of the Boiler Room Operator is of paramount importance. All Boiler Plant Operators on duty during the off hours shall adhere to the following:
During the following shifts 12:00 a.m. to 8:00 a.m. and 4:00 p.m. to 12:00 a.m. including all weekend shifts and Federal holidays, the on-duty operator will carry a two-way radio issued to the Boiler Plant for his safety. If help is needed, the operator should contact the Hines Police
Department via the two-way radio.
The on-duty operator will initiate a call to the Hines Police Department via the telephone at extension 22379 at the beginning of the shift and the middle of the shift to ensure that the ability to perform assigned duties has not been impaired due to accident or other event.
In the event the operator does not contact the Hines Police Department at the beginning or middle of the shift via telephone, the Police Department will initiate a phone call to the Boiler
Room Plant at extension 22387. If there is no answer, the Police will send an Officer to the
Boiler Plant to physically check on the operator.
It is VHA policy that Boiler Plant Operators, Boiler Plant Supervisors and Boiler Plant Service
Managers successfully complete appropriate Boiler Safety Education.
• A 12-part video and A handbook on the boiler safety education program titled "Keep it
Steamin’: Keep it Safe" has been developed by the Employee Education System (EES) as the basic VHA boiler safety education course.
• The 12-part course is mandatory for all VHA boiler operators and their immediate supervisors. Parts 10-12 are mandatory for Chief Engineers, Facility Managers, and Safety
Specialists. NOTE: Nothing in this Directive is to be construed to mean that the 12-part “"Keep it Steamin’: Keep it Safe"” courses are to be the only educational programs for boiler plant staff.
• Ensuring the 12-part course is completed within 6 months of an employee’s assignment to boiler plant operation, boiler plant supervisory, or boiler plant managerial duties.
It is VHA policy that each boiler plant be operated in a safe and economical manner. In compliance with Department of Veterans Affairs (VA) standards and directives and national codes, such as the American Society of Mechanical Engineers (ASME), Boiler and Pressure
Vessel Code and the National Fire Protection Association (NFPA) 85 Boiler and Combustion
Systems Hazards Code.
Education/Training: Engineering Services / Boiler Plant have reviewed this policy to include in-service training.

1. PURPOSE: This Hines VA Hospital; Engineering Service policy on the operation of the
Boiler plant at Hines VA Hospital campus.
2 POLICY: It is Hines VA Hospital policy that the Boiler plant located at Hines campus be operated in a safe and economical manner recognizing its importance and the potential for pressure vessel and/or furnace explosion. Physical plant safety is paramount and is the first priority for Hines VA Hospital Boiler plant operations.
The Boiler plant is essential to the operation of the medical center providing steam and hot water required for sanitation, infection control and a healthful environment for the delivery of health care.
Boiler plant safety is dependent on well-trained operators, properly functioning safety equipment, proper operational procedures, well-maintained boilers and support equipment, and a commitment to continuous quality improvement.
3. PROCEDURES: All safety devices installed on the physical plant boilers, or installed as part of the fuel systems, steam distribution systems and condensate return systems, shall be periodically tested to ensure their proper function in accordance with the VHA Boiler Plant
Safety Devices Testing Manual. Results of the testing will be documented on the Boiler Safety
Devices Sheet. Boiler plant safety is paramount and is the first priority for Boiler plant operations. Boiler plant safety shall not be compromised in order to maintain steam service.
a. Operational rounds are an integral part to Boiler plant safety and are the first line of defense against a catastrophic failure of a boiler safety device. Operational rounds shall be made on the hour by all shifts and shall include a complete cataloging of the major operating equipment. The logbook will be reviewed and initialed by the oncoming operator and filled out throughout the shift with the following information but not limited to;
• Any situation that requires action on the part of the operator other than routine maintenance activities (i.e. trouble calls, disaster drills, fire alarms, bomb threats, etc.)
• Name of caller, time called & reason for call.
• Equipment that is off line due to breakdown or maintenance will be listed with a cause and potential repair date.
b. The safety of the Boiler Room Operator is of paramount importance. All Boiler
Plant Operators on duty during the off hours shall adhere to the following:

• In the event the operator does not contact the Hines Police Department at the beginning or middle of the shift via telephone, the Police Department will initiate a phone call to the Boiler
Room Plant at extension 22387. If there is no answer, the Police will send an Officer to the
Boiler Plant to physically check on the operator.
c. The Boiler plant Operator will review and acknowledge all alarms. All active alarms will be entered into the log when action is required and the action taken. At the change of shift all alarms will be signed over to the oncoming operator who will accept responsibility for the corrective action required.
d. Notification of a non-functioning safety device – Normal Operating Hours: The on-duty operator upon discovery of a non-functioning safety device shall immediately notify the Boiler
Plant Foreman or the Chief of Maintenance and Repair. If the safety device cannot be readily repaired, or replaced, thereby, rendering the boiler out-of-commission. The on-duty operator shall log the findings in the daily log. After Normal Hours of Operation: The on-duty operator shall log the findings in the daily log. The oncoming dayshift operator shall notify the Boiler plant Foreman or the Chief of Maintenance and Repair of the non-functioning device.
e. (At any time where an emergency condition(s) exist that would compromise boiler plant safety, (i.e.) boiler safety device is recognized as non functional) or reduction in steam-generating capability, the on-duty operator shall contact the Boiler Plant foreman for proper authorization to call in a contractor for repairs, and authorization to implement steam load shedding procedures. If it is determined that boiler operations cannot be readily restored, the
Chief of ENGINEERING or the designee will notify the Medical Center Director.
f. Boiler plant operators shall be knowledgeable of the location, purpose, and proper functioning of all physical plant safety devices. Physical plant operators shall at no time leave any high-pressure boiler (above 15psig) unattended except for brief moments to use the restroom. In addition, unqualified persons shall not relieve physical plant operators.
g. Physical plant operators shall participate in an ongoing training program to develop, maintain, and regularly refresh proficiency in safe boiler operations and shall be proficient in properly performing the following:
1) Lighting off, warming up, placing in service, and shutting down the boilers.
2) Firing on each of the available fuels.
3) Operating all plant equipment and controls, including start-up and shutdown.
4) Gradual warm-up of hot piping systems and placing them into service.
5) Maintain water quality to protect the equipment and piping from damage.
6) Handling physical plant malfunctions and emergency situations.
7) Collecting and organizing the plant performance records and logs.
8) Routine equipment maintenance.
9) Preparation of equipment for inspections.

10) Facilitating and monitoring the receipt of fuels and supplies.
11) Generate weekly, monthly, and annual reports as required.
h. Engineering operation of all equipment shall be operated at the highest cost effectiveness and efficiency. This means maintaining steam pressure at the minimum necessary for the proper operation of the plant and connected loads.
i. The Engineering steam conservation program shall focus particularly on maintaining steam traps, condensate pumps, and the integrity of piping systems and pipe insulation. Steam and condensate leaks and other necessary repairs must be reported to the work order desk and given a high priority.
j. Engineering shall develop a steam load-shedding plan for implementation during a boiler plant emergency that reduces steam-generating capability. This plan shall identify the critical loads that must continue to be served, to the greatest extent possible, without compromising
Boiler plant safety.
k. Engineering shall ensure provisions have been made to include emergency electrical generator serving the Boiler plant is included as part of the medical center’s emergency power supply system testing and maintenance program.
l. Engineering shall ensure that the water treatment program include daily tests, the use of chemicals and includes a periodic review by a technical representative of the chemical supplier, by an independent water treatment consultant, or a properly trained operator.
m. Engineering shall develop a utilities systems security program ensuring limited access to the physical plant and on-site fuel facilities, including fuel storage and piping systems.
n. Engineering shall establish a system for regularly checking on the physical plant operator during each shift to ensure that the ability to perform assigned duties has not been impaired due to an accident or other event.
o. Engineering physical plant operators shall have annual physical examinations to ensure physical fitness to perform assigned duties in accordance with VA Handbook 5019, pt. II.
p. Engineering shall develop a standard operating procedure for steam shutdown to allow for maintenance to be performed.
q. All repairs to boilers and pressure vessels at Hines VA Hospital shall comply with the
American Society of Mechanical Engineers (ASME) Boiler Pressure Vessel Code. No welded repairs are permitted, except by a certified welder using Code-approved procedures.
r. Engineering shall ensure that On-Site Reserve Fuel Requirements are maintained to meet the normal demands of continuous operation for a period of 10 January days.
s. It is VHA policy that Boiler Plant Operators, Boiler Plant Supervisors and Boiler Plant
Service Managers successfully complete appropriate Boiler Safety Education.

TESTING
Inspection and Testing are effected appropriately
(1) Qualified Testing Staff. A Qualified Inspector is an individual who is determined by the medical center’s management to be qualified to inspect equipment by reason of training and experience. The required services must be procured from qualified individuals, if no VA staff is qualified:
(a) Hydrostatic testing of boilers and pressure vessels must be conducted after a repair or a tube replacement, or when the integrity of the boiler or pressure vessel integrity is in doubt.
Hydrostatic pressure must be limited to 150 percent of normal operating pressure of the boiler or pressure vessel.
(b) Selections of contractors for plant services including boiler inspections, burner adjustments, testing of safety devices, calibration of instruments, and monitoring of water treatment must be based on quality as the first priority. The contracting method chosen must allow contractors to be selected on the basis of qualifications as a first priority.
(c) A Qualified Professional Inspector (QPI) is any one or combination of:
1. A boiler inspector who has a valid commission from the National Board of
Boiler and Pressure Vessel Inspectors.
2. A boiler inspector who has qualified by passing a written examination under the laws, rules and regulations of a jurisdiction of the state.
3. A boiler inspector who is regularly employed as a boiler inspector by a jurisdiction that has adopted and administers one or more sections of the American Society of Mechanical
Engineers (ASME) Boiler and Pressure Vessel Code as a legal requirement, and has a representative serving as a member of the ASME Conference Committee.
4. A boiler inspector who is regularly employed by an insurance company that has been licensed or registered by the appropriate authority of a state of the United
States to write boiler and pressure vessel insurance.

Note: - VHA requires that certain boiler plant safety devices be provided and requires periodic inspections and tests of boilers/burners and safety devices (VHA Directive 2003-050, “Boiler
Plant Operations”). VHA requirements are based on the major codes, but codes are considered to be minimum requirements. VHA has some requirements that exceed the codes. These requirements are significant safety enhancements.
