About this file

This is a draft pricing schedule attachment for a VA pathology and laboratory services contract at the Nebraska-Western Iowa Health Care System (NWIHCS). The schedule provides comprehensive pricing templates for clinical diagnostic laboratory (CLIA) and professional fee schedule services across two contract years (2027-2028), with estimated quantities and unit pricing based on CMS fee schedules.

The pricing structure covers approximately 360 distinct laboratory tests and procedures organized by service category, including molecular pathology, clinical chemistry, hematology, coagulation studies, immunology, microbiology, cytopathology, and surgical pathology. Offerors must populate unit pricing for each CPT/HCPCS code, specify applicable CMS discount rates (with 100% of CMS pricing as the baseline), and identify which laboratory will perform each test using a numerical designation system (0 for self-performed; 1-26 for subcontracted labs). The schedule requires offerors to complete a separate Laboratory Information section documenting all laboratories' names, physical addresses, CLIA numbers, and laboratory director contact information. Key test categories include high-volume procedures such as HPV high-risk pooled testing (972 units annually), tissue examination by pathologist with technical components (1,075 units), cytopath cell enhancement (576 units), and chromosome analysis (92 units), alongside lower-volume specialized tests. The template also accommodates miscellaneous tests with TBD pricing pending methodology determination. All pricing is structured as fixed-price unit costs per test, with total contract pricing calculated by multiplying quantities by unit prices and applicable discount rates.
