About this file

This document provides the cost response instructions and worksheet for the Request for Responses (RFR) issued by the Executive Office of Health and Human Services (EOHHS) of Massachusetts to procure a vendor or vendors to provide K-12 COVID-19 Testing Services, including pooled and individual diagnostic testing, to authorized schools and other designated programs. The RFR requires bidders to submit pricing for administrative payments, per pooled testing kit, per individual diagnostic test, and for transporting specimens to authorized processing laboratories and the Massachusetts State Public Health Laboratory. Bidders must also provide hourly rates for logistical and support services such as site-based coordinators, specimen collectors, and collection observers.

The pricing must reflect fixed rates and price ceilings, as applicable. Bidders are required to follow specific naming conventions for the cost response template and to provide additional notes on pricing, if necessary. This RFR is part of the 21EHSKAK-12COVID19TESTSVCS state and local contract opportunity in Massachusetts.
