About this file

The document is a set of three annual affidavits from the Commonwealth of Kentucky, specifically for bidders, offerors, and contractors participating in state contracts. These forms are required by Kentucky Revised Statutes (KRS) 45A.110, 45A.115, 45A.465(3), and 45A.494(2), and are valid for one year from the date of execution. The affidavits cover three specific areas of bidder qualification: general campaign finance compliance, qualified bidder status (particularly for nonprofit agencies serving individuals with severe disabilities), and resident bidder status.

The affidavits require bidding entities to swear under penalty of perjury that they meet specific legal and operational requirements. For resident bidder status, companies must be authorized to do business in Kentucky, have filed Kentucky corporate income taxes, made payments to the Kentucky unemployment insurance fund, and maintained a Kentucky workers' compensation policy for one year prior to bidding. The forms also allow entities to claim special status, such as being a nonprofit corporation or a qualified nonprofit agency for individuals with severe disabilities. The bidding agency reserves the right to request documentation supporting these claims, with failure to provide such documentation potentially resulting in disqualification or contract termination.
