VENDOR QUALIFICATIONS: Please reply to each numbered item below.

1. Vendor shall provide documentation demonstrating they are properly licensed and insured.

2. Vendor shall describe in depth their experience in large vehicle transportation and provide references, including a point of contact and phone number, supporting their past performance in transporting large vehicles.

ATF RESPONSIBILITIES:

It will be the responsibility of ATF to notify the vendor within ten (10) days that a truck is ready for pickup. Due to the nature of the NRT trucks, Vendor will be notified of the truck’s location three (3) days prior to pick up.

ATF will document the condition of the vehicle at pickup and delivery.

ATF will look at reliability of the vendor.

Because each NRT truck will be delivered at different times, Vendor will be paid following the delivery of each truck after submitting invoice to ATF.

COMPLETION CRITERIA:

• Work will be considered complete when the vehicle is delivered to its final destination.

• ATF will verify that each NRT truck is in the same condition as when it was picked up from the original location.

• Once the NRT truck is deemed to have arrived at its final location and the condition of the truck is the same as when it was picked up, Vendor will submit an invoice for that delivery and be paid.
