STATEMENT OF WORK (SOW)
This is a Bulk-Funded Blanket Purchase Agreement (BPA) for the purchase of succinylcholine chloride injection 100mg/5ml (20mg/ml) vials, phenylephrine 1mg/10ml (100mcg/ml) 10ml vials and Oxytocin 30 units added to Lactated Ringers 500 ML in 500 ML excel bag on an “as needed basis” required by the Gallup Indian Medical Center Pharmacy at Gallup Indian Medical Center (GIMC) in Gallup, New Mexico 87301. Products must be sterilely compounded in an FDA approved facility with a beyond use date of at least 90 days from manufacture for succinylcholine chloride and phenylephrine injections and 30 days from manufacture for oxytocin.
(1) The Bulk-Funded amount as specified is an estimate only and may not be fully expended. If the Government’s requirements do not result in the estimated amount as stated on the Bulk Funded BPA amount, that fact shall not constitute the basis for an equitable price adjustment.
(2) The Contractor shall provide Government pricing or better to the Gallup Indian Medical Center, Gallup, NM.
(3) If the Government requires urgent delivery of any quantity of an item before the earliest date that delivery may be specified under a call order, and if the Contractor will not accept an order providing for the accelerated delivery, the Government may acquire the required goods from another source.
(4) Individual orders shall not exceed the simplified acquisition threshold of $250,000.00.
(5) For a particular purchase greater than the micro-purchase threshold of $10,000.00 and above, the government reserves the right to ensure maximum practicable competition, by soliciting quotations from other sources and to make the purchase as appropriate.
(6) The following individuals are hereby authorized to place Call Orders, within their dollar limitation, under this order:

(7) THE PERIOD OF PERFORMANCE FOR THIS BPA IS AS FOLLOWS:
· Base Year: January 1, 2023 through December 31, 2023 with four (4) one-year option periods
(8) Any Call Order issued during the effective period of this BPA and not completed within that period shall be completed by the Contractor within the time specified in the Call Order.
(9) All deliveries under this BPA must be accompanied by packing slip that contains the following information as a minimum:

| (c) Catalog and National Drug Code (NDC) Number; | |
| (d) Delivery Order Number; | |
| (e) Date of Purchase; | |
| (f) Itemized list of supplies, Quantity, Unit Price, and Total Amount for each item; and | |
| (g) Date of Shipment. |
Packing slips shall contain the Bulk-Funded Purchase Order Number for immediate identification by the GIMC warehouse/receiving personnel.
