Statement of Work for Elevator Maintenance and Repair

1.0 Introduction: The Butler VAMC operates four (4) elevators with a total of twenty-two (22) stops and doors. The elevator types consist of three (3) electric (traction) and one (1) hydraulic types. The vertical transportation at this facility plays a part in providing services to our Veterans and must do so without interruption. The reliability and safe operation of the vertical transportation equipment must be maintained to ensure reliability and safe operation. The Butler VAMC is seeking a qualified contractor with elevator maintenance and repair experience, that can meet the requirements of this document.

2.0 Purpose

2.1 The purpose of this Statement of Work (SOW) is to obtain maintenance and repair services, safety inspections and testing for Vertical Transportation Equipment (VTE) at the Butler Veterans Affairs Medical Center.

2.2 This is a full maintenance, inspection, repair and testing contract as covered in CLIN 0001, 1001, 2001, 3001, 4001, in which the elevator service provider (Contractor) takes total service responsibility for the equipment identified in the contract. As such, the monthly firm fixed price includes all service, testing and inspections requirements, maintenance, repairs, parts replacements (up to $2,500) and routine or emergency service calls including premium time not covered in 2.3 below. This allows the Government to budget total yearly costs and eliminates concerns relating to individual parts replacement, emergency repairs and premium hour calls.

2.3 Emergency Services, parts replacement and extra items, as covered in CLIN 0002, 1002, 2002, 3002, 4002, are those items covered in paragraph 5.0 and 10.3.2 below.

2.3.1 Emergency services are those items defined in paragraph 2.4 below. The Government reserves the right to obtain repair services from other vendors.

2.3.2 Prior to engaging in service or repair under paragraph 2.2 the contractor shall obtain written approval from the CO or the Government POC/COR. Email is an acceptable form of written approval. Any work performed without CO or COR/Government POC approval shall not be approved for payment and considered and Unauthorized Commitment.

2.4 Emergencies defined: CO or Government POC/COR will have the final call as to what is considered emergency work. Any work performed outside of normal business hours, 7:30 am -4:00pm Monday thru Friday. This does not cover work started during normal business hours but not completed by the end of those normal business hours. COR or Government POC.

3.0 Codes and Standards

3.1 The following publications listed form a part of this Contract. Elevators shall meet the requirements of the most recent code editions published on or before the date of award of the Base Contract or at the time of award of each successive Option.

4.0 General Requirements

4.1 Contractor shall provide all management, supervision, labor, parts (regardless of obsolescence), materials, tools, incidental engineering services, testing services, transportation and all other appurtenances, including overtime pay, weekend and holiday premium pay, required for the maintenance and repair of the vertical transportation equipment in a continual operating status as originally specified by the equipment manufacturer and/or as modified when a deviation is required but only with the written consent of the VA.

4.2 Contractor, without additional expense to the Government, shall obtain all licenses, permits, and insurance required to perform the work of this Contract. Contractor shall comply with all applicable federal, state, local and industry codes and laws.

4.2.1 Evidence of insurance, licenses, and permits shall be provided to the Contracting Officer before prior to start of contract performance.

4.3 The Contractor must be able to respond within one hour to all service calls (to include TRAP CALLS) at no extra cost to the Government. For responses to non-trap calls the contractor shall be on-site within two hours of receiving the initial request. Trap calls require on-site service within one hour.

4.4 The contractor shall provide Cloud based Customer Service Portal.

4.5 The contractor shall provide 24-hour 365-day dispatching service to ensure quick response.

4.6 The Contractor is required to sign in at the fire station on all visits including routine maintenance and service calls.

4.7 Contractor shall provide tools, equipment, specialty items and other items that may be necessary to perform the tasks required under this Contract.

4.7.1 Any parts not available from the manufacturer due to obsolescence remain the Contractor’s responsibility to replace from another source.

5.0 Exclusions to routine maintenance: The subparagraphs below further clarify the scope of work under this contract. The only exceptions to performing this work under the scope of this contract for routine maintenance is if this work is deemed an emergency and therefore would be performed as defined in 2.4.

5.1 For the purpose of clarification, any item not specifically excluded shall be considered the Contractor’s responsibility and therefore covered under the routine maintenance of the contract.

5.2 Equipment, components, and parts not installed under this contract that are under the manufacturer’s or installer’s warranty.

5.3 Installation of new attachments or equipment not currently part of the VTE at the time of award of the Base Contract that is required by inspectors, insurance agencies or VA/Government Authorities.

5.4 Repair or replacement of elevator equipment due to natural disaster, vandalism, negligence, or misuse by persons other than the contractor, its representatives and employees unless any of those items create an emergency. In the event they result in an emergency they shall be handled in accordance with paragraphs 2.2 and 2.3 of this SOW. The Contractor shall obtain written approval by the CO/COTR prior to conducting related repairs. Payment will not be made for any unauthorized work.

5.5 Replacement of underground hydraulic piping or hydraulic cylinder.

5.6 Cleaning, refinishing, repair, and replacement of cab enclosures, hoistway enclosures, door frames, and cab flooring.

5.7 Repair or replacement of elevator pit sump pumps, machine room and hoistway light fixtures, heating/ventilation/air conditioning systems (HVAC), fire/smoke/heat alarm systems and sprinkler systems.

5.8 Power supplies from the elevator main service disconnect panel to its source.

5.9 CO/COTR must approve in advance all repairs not covered under this contract.

5.9.1 The Contractor shall prepare an estimate of parts, labor, transportation, and shipping costs.

5.9.2 If approved, CO/COTR shall issue a separate Purchase Order (PO) in advance of undertaking such work. No advanced promise that the contractor will be awarded this work.

5.9.3 No invoice will be paid without prior approval and specific PO issued in advance of work.

5.10 There shall be no borrowing of parts (i.e., removing a part off of an elevator unit, whether in it is in service or not, to correct a deficiency on another elevator unit) except in an emergency (i.e., entrapment, etc.) or with written consent from the VA.

6.0 Safety

6.1 The following practices shall be observed, at a minimum, during maintenance, inspection, or testing procedures:

6.1.1 All safety devices must be in operational condition.

6.1.2 Lockout/tagout procedures must be followed if maintenance procedures require that the equipment not be operated.

6.1.3 Ensure that personnel performing maintenance, inspection, and testing tasks wear clothing that is not loose fitting and that they are provided with proper protective equipment, such as safety shoes, hard hats, eye protection, and hand protection.

6.1.4 Provide barriers and signage, where applicable, especially at hoist way doors.

6.1.5 Upon completion of work, remove any jumper wires that were used.

6.1.6 It is possible that the elevator pit may be designated a "Permit Required Confined Space." The additional required safe procedures must be attended to in these cases.

6.1.6.1 Provide proper lighting.

6.1.6.2 Determine that adequate refuge space exists above and below the car.

6.1.6.3 Ensure the working area is clean and dry.

6.2 More detailed safety procedures can be found in publications such as the Elevator Industry Field Employees’ Safety Handbook.

6.3 Contractor shall adhere to all OSHA, EPA, NFPA Life Safety Codes, and all other regulatory requirements.

6.4 In performance of this contract, the Contractor shall follow their Corporate Safety Policy and standards for safe work practices and shall also take such reasonable safety precautions as the VAMC Safety Officer or designee or the CO/COR may determine to be necessary to protect the lives and health of occupants of the building. The Contractor shall comply with VAMC smoking policy, which designates all interior space as NO-SMOKING areas.

6.5 The Contractor shall treat all elevator pits as “Permit Required Confined Space” and shall follow all OSHA requirements and VAMC requirements when entering such spaces.

6.5.1 Elevator pits may be reclassified as a Confined Space as outlined in VA policy 138-08-S5; such policy shall be followed carefully. VAMC will provide special training to Contractor on expectations of this policy which will include completion and posting of the permit every time an elevator pit is entered.

6.6 Patient, employee, visitor, and Contractor personnel safety shall be maintained at all times.

6.6.1 The Contractor is responsible for the safety and health of his/her employees.

6.6.2 The Contractor shall comply with all applicable OSHA safety and health standards.

6.6.3 The Contractor shall replace all safety guards, equipment, devices, etc. removed to service or repair the elevator immediately after completion of work or when leaving the job unattended.

6.7 The Contractor shall submit to the Contracting Officer, prior to the start of the contract, the Material Safety Data Sheets (MSDS) for all potentially hazardous materials (lubricants, cleaners, fluids, etc.) to be used in the facility in performance of the contract, and will not use, in the facility, such materials which have not been so cleared in advance with the VAMC.

6.7.1 MSDS for new chemicals shall be furnished prior to the arrival of the chemical on site.

6.7.2 The Contractor shall maintain a copy of all MSDS in each elevator machine room to assure compliance with all laws and requirements regarding the “Right to Know” law.

6.8 Contractor is responsible to identify, provide and maintain all personal protective equipment (PPE) required to perform the duties outlined in the contract.

6.8.1 The Contractor is responsible for identifying and providing all applicable safety programs required to perform the work i.e., lockout/tag out, confined space entry, universal precautions, etc.

6.8.2 Training required for safety programs and the proper use of PPE shall be provided by employer, and documentation maintained by the Contractor.

6.9 The Contractor shall obtain a Hot Work Permit from the Safety staff or the CO/COTR in advance whenever soldering, welding, grinding, and using a cutting torch, or other open flame, spark or heat producing equipment is used.

6.9.1 The Contractor is required to follow all requirements outlined for the issuance of the Hot Work Permit.

6.10 The Contractor shall be notified of any non-compliance with safety regulations.

6.10.1 The Contractor shall, after receipt of such notice, immediately correct the condition and notify the Contracting Officer in writing that the condition has been corrected.

6.10.2 If, in the opinion of the CO/COTR or the VAMC Safety Officer the condition is life threatening he/she shall instruct the Contractor to stop work until the condition has been corrected. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order halting all or any part of the work, and the Contractor may be held in default. The Contractor bears all costs of stop-work time and correcting safety hazards.

6.11 The Contractor shall reinstall guards or equipment panels, removed to perform maintenance, on the equipment before putting it back into service.

7.0 Inspection and Testing

7.1 ASME 17.1 Category One, Category Three and Category Five tests shall be performed by the Elevator Contractor, including three- and five-year load testing, as part of this Contract.

7.1.1 The Contractor provided testing shall be witnessed by an ANSI/ASME QE-1 Certified Elevator Inspector Contracted separately by the VAMC.

7.1.2 The Government shall coordinate the inspection with the third party inspection contractor.

7.2 The Contractor shall perform all safety tests and other tests and inspections as recommended and required by ASME A17.1 “Safety Code for Elevators and Escalators” using Table N-1 “Recommended inspection and test intervals in ‘Months’” and ASME A17.2 “Inspectors’ Manual for Electric Elevators” latest edition on all equipment listed in paragraph 19.0 of this document.

7.3 The certified inspector will provide a report of all deficiencies found that the VAMC or elevator contractor is required to correct.

7.4 The results of ALL tests shall be recorded in the “Record of Elevator Testing” and a summary delivered upon request to the CO/COTR.

7.5 Any repairs or adjustments necessary to complete a test and return the elevator to service shall be performed by the Contractor at no additional expense to the Medical Center in accordance with paragraph 2.2 of this document.

7.6 The Contractor shall attach tags after testing as required by ASME A17.1 and A17.2.

7.7 All elevators provided with firefighter’s service shall be subjected to monthly Phase l “Emergency “Recall Operation” and a minimum of one floor operation on Phase ll “Emergency In-Car Operation” to verify the system is operating order in accordance with ASMEA17.1.

7.7.1 The contractor shall conduct these tests as required and the results shall be recorded in the MCP under Record of Elevator testing and a copy provided to the facility Safety Manager and a companion separate document maintained in the elevator machine room.

7.7.2 These tests are to be scheduled with the Safety Manager and must be performed before or after the WBVAMC normal business hours of 8:00AM – 5:00PM.

7.8 All tests or inspections that will remove the elevator from normal operation shall be scheduled in advance with the CO/COTR.

7.8.1 Recurring tests such as “Emergency Recall Operation” may have a standard recurring schedule approved and applicable through the term of this agreement.

7.9 The Contractor shall perform necessary inspections and tests as required under ASME 17.1 and ASME 17.2 using a Qualified Elevator Inspector (QEI). The third-party inspector is at the Government’s cost but the contractor shall cover the expense of items needed for the inspection and tests (i.e. properly marked testing weights, voltmeter, center reading ammeter, thermometers, stopwatch, and direct reading tachometer).

7.9.1 The (5)-year test for all traction and hydraulic elevators must be completed within three (3) months of award of this contract regardless of due date.

8.0 Injury And Accident Reports

8.1 The Contractor shall immediately report to the CO/COR in a manner and on the forms prescribed by the VAMC all accidents resulting in injury, trauma, death, hazardous exposures, and occupational disease involving an elevator or its maintenance.

8.2 Written reports for all incidents shall be provided within five (5) working days and include the cause, what, where, and when the incident occurred as well as repairs and tests performed to correct the cause of the incident.

9.0 Infectious Control and Cleaning

9.1 The VAMC maintains the highest level of cleanliness as a part of its Infectious Control Program to ensure that patient health is not compromised. As such, the Contractor shall maintain the elevator shafts, elevator pits, equipment rooms and equipment room access to these standards and comply with all other reasonable requests of the CO/COR related to Infectious Control measures.

9.2 Within the first 30 days of award of the base contract or Contract Options, the Contractor shall thoroughly clean the following: all elevator hoist ways, pits, cartops, machine rooms and equipment including controller interiors and air filters. All car and hoist way door hardware shall be cleaned, lubricated, and adjusted.

9.3 All floors including elevator pits and steps shall be swept, mopped, and vacuumed using a HEPA vacuum cleaner as often as necessary to prevent accumulation of dirt, oil, and grease.

9.4 All walls, ledges, handrails, storage cabinets, fences, elevator shaft walls, and other surfaces shall be kept clean using a HEPA vacuum cleaner and damp cloths. Cleaning shall be performed as often as needed to remove dust and dirt from surfaces.

9.5 Elevator equipment, including control cabinets, motors, door operating mechanisms, and cartops shall be wiped down with damp cloths and vacuumed with a HEPA vacuum as to remove any dust or dirt accumulation. Before cleaning is performed ascertain if any build up indicates a problem with the equipment and requires service or repair.

9.6 The exterior of the machinery and any other parts of the equipment subject to rust shall be properly painted. Any surfaces, particularly floors, steps and handrails that have a dull or worn appearance after cleaning shall be painted so that cleaning efforts are evident.

9.7 CO/COR shall periodically inspect equipment and equipment spaces for cleanliness. The inspection shall consist of a visual observation to detect any accumulation of dust on any surface required to be kept clean under this Contract.

10.0 Maintenance and Repairs

10.1 The Contractor shall provide all parts, equipment tools and labor required to complete maintenance service, safety inspections, and required tests per ASME 17.1, ASME 17.2 current editions and Pennsylvania Department of Labor and Industry Laws and Regulations on each elevator at the Butler VAMC (see paragraph 19.0) for the period required under the Contract.

10.2 The Contractor shall not make any alteration or additions to the equipment without prior written approval from the CO.

10.3 The Contractor shall note that if work is required that is outside of the scope of this contract.

10.3.1 The Contractor shall submit a detailed cost estimate for the required repairs to the COR.

10.3.2 The Contractor shall not proceed with such work, except for an emergency such as entrapment, without the written consent of the CO and receipt of a separate Order to cover the cost of the additional work. Anything over the micro purchase threshold shall be vetted through the COR regardless of status.

10.4 The contractor is required to develop a written Maintenance Control Program (MCP) specific to the vertical transportation equipment listed in paragraph 19.0 and in compliance with ASME 17.1, ASME 17.2, and manufactures requirements.

10.5 Within Thirty (30) calendar days after contract award the Contractor must submit to the Maintenance and Operations Supervisor a copy of the written MCP. The MCP must be specific to each vertical transportation system at the Butler VAMC.

10.6 The MCP shall always be viewable onsite by elevator personnel.

10.7 The MCP shall include but not limited to ASME 17.1 maintenance tasks, maintenance procedures, and examinations and tests listed within ASME 17.1.

10.8 The MCP must be kept current with the latest edition of ASME 17.1.

10.9 The MCP shall include the equipment manufacturers maintenance procedures and required frequencies and checklist with required maintenance for each elevator.

10.10 The MCP checklist must be kept current and located in the relative machine room for each vertical transportation unit.

10.11 The Contractor shall provide lubricants, fluids, fasteners, cleaning supplies, paints (low VOC only), and all other supplies necessary to perform the services required in this Contract. 10.11.1 Lubricants shall be only those products recommended by the manufacturer of the equipment or equal.

10.12 The contractor is responsible for the cleaning as follows.

10.12.1 Car tops, machine room floors, and machine room equipment shall be cleaned monthly.

10.12.2 Accumulated rubbish shall be removed from the pits monthly.

10.12.3 A general cleaning of the entire installation including all machine room equipment and hoistway equipment shall be accomplished annually.

10.12.4 Cleaning supplies and a HEPA vacuum cleaner shall be furnished by the Contractor.

10.13 Contractor shall perform maintenance in accordance with the MCP to ensure reliable and continuous safe operation.

10.13.1 The maintenance work shall be in accordance with ASME 17.1, commercial practices and manufacturer’s requirements.

10.13.2 The maintenance performed shall be intended to maintain the elevator in safe and reliable operating condition.

10.13.3 The MCP is required to specify appropriate intervals for the specific maintenance items required.

10.14 The contractor is responsible for developing a schedule that will allow enough time to allow the required maintenance to be completed.

10.14.1 The contractor must provide a copy of the scheduled maintenance and inspection procedures to the Maintenance and Operations Supervisor.

10.15 The Contractor shall maintain an adequate supply of spare parts shall be maintained at all times to ensure prompt preventive maintenance and repair services.

10.16 The Contractor is responsible for replacement of all light bulbs, including lights in elevator shafts on top and bottom of cars, HOISTWAY crawl spaces. This shall include all lights inside the elevator cabs.

10.17 The Contractor shall during regular PMI visits retrieve any objects such as keys, wallets, glasses, etc. dropped in elevator pit by occupants of the building. Such objects shall be turned over to the VA Police personnel.

10.18 Elevator Out Of Service

10.18.1 Notify Boiler Plant (After-Hours), Police (After-Hours) and the COR during normal hours, either by phone or in person, before removing an elevator from service. If work requires more than one day, daily notification will be made. Notify Facilities Management Service and the COR when placing the elevator back in service.

10.18.2 When an elevator is taken out of service, appropriate signage shall be installed at each entry door to notify elevator users of the interruption. Contractor shall provide signage and post in a manner which does not damage paint or other finishes. Upon completion, all signage shall be immediately removed.

10.18.3 All tests, inspections, or maintenance repairs that will remove an elevator from service shall be scheduled in advance with the COR.

10.18.4 Under no circumstances will any shutdown or breakdown last longer than five (5) consecutive working days, from initial notification to the Contractor, without prior approval of the COR.

10.19 Performance Standards

10.19.1 Contract speed and brake to brake flight time shall be maintained as originally installed and adjusted.

10.19.2 Maintain smooth starting, stopping, and leveling accuracy of 3mm (1/8”) for all elevators.

10.19.3 Opening times, closing times and door close torques of all elevators shall be maintained within the limits of ASME A17.1 Code but shall be adjusted with a longer hold open and slower closing time to accommodate elderly and disabled patients.

10.19.4 Door reversal on all elevators equipped with mechanical safety shoes shall always be initiated with the stroke of the shoe. Ensure all electric eyes or door detectors are working at all times.

10.19.5 Variable car and hall door open times shall be maintained in accordance with original field adjustments. Deviations from this will not be permitted unless requested and/or approved by the CO/COR.

10.19.6 Contractor shall ensure that the original operating criteria is maintained at all times for each elevator including, but not limited to, the following:

10.19.6.1 Maximum capacity in pounds;

10.19.6.2 Rated speed in feet per minute;

10.19.6.3 Performance time measured brake to brake;

10.19.6.4 Door operation;

10.19.6.5 Traffic handling capabilities;

10.19.6.6 Response times;

10.19.6.7 Ride quality.

11.0 Maintenance, Repair, And Callback Tickets

11.1 The Contractor shall have all maintenance tickets signed by the CO/COR.

11.1.1 In the event of after-hours maintenance, Contractor shall have the maintenance ticket signed by the Administrative Officer of the Day (AOD).

11.2 The Contractor shall instruct his personnel that when they perform work under this Contract, the shall comply with the following procedures:

11.2.1 Upon arrival at the facility, Contractor shall check in with the fire station .

11.2.2 . Pick up the key ring and sign in at the Boiler Plant. The keys will access any lock on the elevators and open access doors to elevator maintenance spaces. This key ring shall be returned before maintenance personnel leave the VAMC.

11.2.3 Contractor shall have a service ticket on each service call signed by Maintenance and Operations Supervisor.

11.2.3.1 Service tickets shall identify the respective piece of equipment, description of problem, description of corrective measures taken, date and time unit was returned to regular service.

11.2.3.2 Signed maintenance, repair, callback tickets are required to verify hours on site. Without signed tickets hours billed will not be paid.

12.0 Quality Control Program

12.1 The Contractor shall establish a complete Quality Control Program to assure the requirements of the Contract are provided as specified.

12.1.1 The Contractor shall submit this program to the CO/COTR, for acceptance within ten (10) days of Notice to Proceed.

12.1.2 The QCP shall be in compliance with the Maintenance Control Program as required by ASME 17.1.

12.1.3 The program shall include the following:

12.1.3.1. An inspection system covering all the services described in the Contract. A checklist used for inspecting Contract performance during scheduled or unscheduled inspections and the name(s) of the individual(s) who will perform the inspection.

12.1.3.2 The checklist shall include every area of the Contractor’s operation as well as every task required to be performed.

12.1.3.3. A system for identifying and correcting deficiencies before the level of performance becomes unacceptable and/or the Medical Center inspectors point out the deficiencies.

12.1.3.4 A file of all inspections conducted by the Contractor and the corrective action taken. This documentation shall be made available to the VAMC upon request, at any time during the term of the Contract.

13.0 Record Keeping

13.1 The Contractor shall maintain a complete set of currents, legible schematic wiring diagrams in each elevator contained therein.

13.1.1To the extent that any of the required schematic wiring diagrams are not available at the time of contact award, it will be the responsibility of the Contractor to provide same at no cost to the Medical Center.

13.2 The Contractor shall maintain a full, legible service log in each machine room and a complete, orderly chronological file including drawings, complete part lists, and copies of all service reports.

13.2.1 The log shall list the data and time of all maintenance service performed and signature of technician.

13.2.2 Each trouble call shall be fully described, including the nature of the call, necessary service performed, and any parts replaced.

13.2.3 Service file shall be made available for inspection upon request, and a copy of the complete file furnished to VAMC during the first week of the last month of the normal contract term.

13.3 The Contractor shall maintain a separate maintenance record on each elevator.

13.3.1 The maintenance record shall be kept in the elevator equipment room and shall be credit readily available for inspection by the COTR or designee.

13.3.2 The maintenance records are the property of the government and shall be turned over to the Government at the end of the contract term.

13.4 Upon each departure, the Contractor will serve notice of disposition of work to the COTR or designee.

13.4.1 In additional the contractor shall furnish the Maintenance/Operations Supervisor or his designee a copy of a work order, service sheet or other such written notification of services performed. Such reports shall include the following information.

1. Name and address of Contractors

2. Name of Contractor employee(s) performing the work.

3. Date(s) work performed and hours

4. Brief description of work performed, including PM inspection identification of equipment worked on.

5. Signature of contractor’s employee(s) and signature of the Maintenance and Operations Supervisor or designee.

6. If the report is not submitted it will be indicated that no inspection or maintenance was performed, and payment may be withheld and/or delayed.

14.0 Work Hours Defined

14.1 Regular time is defined as Monday through Friday 7:00 AM – 3:30 PM excluding weekends and federally observed holidays.

14.2 After hours is defined as the hours between 3:30 PM – 7:00 AM including weekends and federally observed holidays.

15.0 Contractor Qualifications

15.1 Contractor shall have a minimum of ten (10) years of successful experience maintaining elevator equipment identical or similar to the vertical transportation equipment listed in this contract.

15.2 The Contractor shall have at least 10 mechanics and adjusters on staff.

15.3 The Contractor shall provide a Contract Manager who shall be responsible for the performance of the work.

15.3.1 The name of the Contract manager and an alternate shall be designated in writing to the CO/COTR prior to the Contract start date.

15.3.2 Changes to personnel shall be submitted to the Government within five (5) work days of the changes.

15.3.3 The Contract Manager or alternate shall have full authority to act on all Contract matters relating to the daily operation of the Contract.

15.3.4 The Contract Manager or alternate shall be available to speak/meet with the CO or COR within 24 hours of request during normal operating hours, and or within the next business day after Friday or a Federal holiday.

15.4 The contractor shall provide Elevator Mechanics/Technicians who have completed an Apprenticeship Program and Mechanic/Technician Examination approved and certified by the U.S. Department of Labor.

15.4.1 Certified Elevator Mechanics/Technicians shall have technical qualifications of at least five (5) years of successful experience in the elevator industry.

15.4.2 Apprentices shall be actively pursuing Certified Elevator Mechanic/Technician status. Certificates shall be submitted to the CO/COR.

15.5 All on-site employees of the contractor shall have completed the 10-hour OSHA Safety course. Certificates shall be submitted to the CO/COTR

15.6 The selection, assignment, and management of Contractor’s employees are the responsibility of the Contractor.

15.6.1 The Contactor shall not employ any individual identified as a potential threat to the health, safety, security, and general wellbeing, or operation of the VAMC facility or personnel.

15.6.2 The Contractor is fully responsible for the performance and conduct of his/her employees at all times.

15.6.3 The Contractor shall not allow any employee to perform work under this contract while under the influence of alcohol, drugs, or any other incapacitating agent.

16.0 Security and other associated requirements

16.1 Contractor and Subcontractor personnel that work on any VAMC property must consent to Background Checks or shall be denied access to the VAMC facilities. Information required to conduct a Background Check includes: full name, driver’s license number and/or social security number, date of birth, and completion of a background check questionnaire

16.2 Contractors shall ensure their employees and those of their Subcontractors have the proper credentials allowing them to work in the United States.

16.2.1 The Contractor shall not be entitled to any compensation for delay or expenses associated with this requirement and are not excused from proceeding with the contract.

16.3 Department of Veterans Affairs requires all Contractor employees to have a VAMC issued Identification Badge (Flash Badge) and wear it at all times while at the VAMC facilities.

16.3.1 This ID badge shall be issued by the VAMC security office under its rule for issuance.

16.3.2 When an employee leaves the Contractor’s service, or the contract is terminated, the employee’s badge shall be returned to the VAMC immediately.

16.4 Contractor personnel shall present a neat appearance and be easily recognized as a Contractor employee by wearing distinctive clothing bearing the name of the company and a VAMC issued ID badge.

16.5 The Contractor shall instruct his personnel that when they perform work under this contract, they shall comply with the following procedures:

16.5.1 Upon arrival at the station, the Contractor shall check in with the fire station.

17.0 Miscellaneous

17.1 VAMC will furnish the following services at existing outlets, for use in those facilities provided by the VA/VAMC and as required for the work to be performed under this contract: electricity, fresh water, sewage service, and non-hazardous refuse collection (from existing collection points).

17.2 Disposal of equipment and construction debris are the responsibility of the contractor.

17.3 VAMC will provide a small office cubical space for use by the Contractor’s onsite employee. The contractor shall provide their own computer, cell phone and supplies as necessary.

VHA Supplemental Contract Requirements for Combatting COVID-19

1. Contractor employees who work in or travel to VHA locations must comply with the following:

a.Documentation requirements:
1)If fully vaccinated, contractors shall show proof of vaccination.
i.NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.
2)If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test that includes an affiliated telehealth service.

3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.

4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.

a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.

4. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
