About this file
This Statement of Work (SOW) details the procurement of eye clinic equipment for the Hershel "Woody" Williams Veterans Affairs Medical Center (VAMC) at the new Prestonsburg Community-Based Outpatient Clinic (CBOC). The acquisition includes 28 specific line items of specialized optometry equipment, such as Reliance patient chairs, phoropters, various Volk diagnostic lenses, examination tools like a Luneau Retinoscopy Rack, Marco Brightness Acuity Tester, Welch Allyn handles, Reichert AVIA Tonopen, and installation services. Each item has a specific local stock number and requires one unit at an unspecified price.
The SOW emphasizes that these equipment pieces are essential for conducting routine eye exams at the new clinic location. Key contractor requirements include providing a price quote for all listed items, coordinating with the VA Medical Center's biomedical engineering department for hardware installation, offering a warranty with ongoing support, and being an authorized distributor of the products. The contractor must also provide evidence of equivalency if proposing alternative "equal to" products. The place of performance is the new eye clinic located at 236 North Highland Avenue in Prestonsburg, Kentucky.
