VA Pittsburgh Healthcare System (VAPHS) Interior Design Support Services
Statement of Work
Purpose
The VA Pittsburgh Healthcare System (VAPHS) has a requirement for all labor, materials, transportation and supervision to provide support services to VAPHS Interior Design team. Services shall include furniture installation and removal to support area furniture and related equipment reconfiguration projects. Projects support services shall vary based on VAPHS needs.
The Contractor shall pull government owned inventory, deliver, install, and reconfigure existing inventory at all both VAPHS main campuses at University Drive, in Oakland, PA, and H.J. Heinz, in Aspinwall, PA, and VAPHS leased spaces within a 60-mile radius. VAPHS is seeking an indefinite quantity contract for one (1) year from date of award with four (4) one-year options periods.
The Contractor shall provide services on an as needed basis as project support demand will vary based on VAPHS needs. The Contractor will schedule the required level of labor and number and type of vehicles needed to meet the demand of each project. A substantial obligation of funds will be required at the start of each year of the contract, but project frequency will dictate if additional funds will be required.
General Requirements The Contractor shall provide services at the following locations:

0. H. John Heinz III Medical Center – 1010 Delafield Rd., Pittsburgh, PA 15215
0. Beaver County VA Clinic – 300 Brighton Ave., Suite 110, Rochester, PA 15074
0. Belmont County VA Clinic – 67800 Mail Ring Rd, Ohio Valley Mall, Suite 215, St. Clairsville, OH 43950
0. Fayette County VA Clinic – 627 Pittsburgh Rd, Suite 2, Uniontown, PA 15401
0. Washington County VA Clinic – 85 West Beau St., Crossroads Center, Suite 200, Washington, PA 15301
0. Westmoreland County VA Clinic – 5274 Route 30, Suite 10, Greensburg, PA 15601
0. Monroeville, PA VA Clinic (once build out and operations begin in late 2023)
The Contractor will pull interior design and furniture items stored on VAPHS property. The service locations are located at Building 49, H.J. Heinz campus, 1010 Delafield Rd. Pittsburgh, PA 15215 – 1 Aisle within building 49 the warehouse, (6) temperature-controlled trailers, (3) Conex Boxes. University Drive Building 1 and Building 30: various areas within the facility.
The Contractor shall be knowledgeable of the following product lines and be able to determine the various parts and pieces of the inventory and installation for:

0. Allsteel
0. DIRTT
0. National
0. Nemschoff seating
0. Screen Flex paneling
The Contractor shall be available to begin providing services within 10 days of the contract award date.
The Contractor shall provide supervisory-level support to the Contracting Officer’s Representative (COR) during normal business hours. Support shall be on-site if the COR deems it necessary to complete certain projects. If the support is off-site, the Contractor shall be required to provide on-site support within two hours of COR request. The need for on-site supervisory support shall be determined during the planning phase of each project.
After an installation, the Contractor shall provide a list of items that were returned to the H.J. Heinz Building 49 warehouse.
The Contractor shall provide labor for a monthly clean-up of the warehouse and storage trailers. Clean-up days shall be coordinated and scheduled with the COR.
All projects shall have a supervisor assigned that holds a current OSHA 30 Card certification and knowledge of Infection Control Risk Assessment (ICRA) Containment requirements when determined necessary by the COR.
The Contractor shall provide the following project services:
0. Project planning assistance to VAPHS Interior Design Team to develop and generate specifications for area furniture and related item reconfigurations.
0. Sufficient levels of skilled and general labor to support project asset disassembly, removal of discarded or unserviceable items, movement, reassembly and installation of assets at other locations.
0. Appropriate levels and types of vehicles, including vehicle operators, required to support asset movement.
0. Scheduling and delivery of assets on hand, and transportation of assets between all VAPHS locations.
0. All necessary tools, equipment and materials needed to load and unload assets, install and secure items and properly dispose of all associated packing/crating materials.
The Contractor shall provide and be knowledgeable in using the following tools and vehicles:
0. HEPA Drills with Dust extraction tube kit, with hose
0. HEPA Shop VAC that shall allow the attachment of a dust extraction attachment
0. Hammer Drill
0. Pallet jack, dolly and related common moving equipment
0. Vehicles – Van, Standard Box Truck and 20’ Box Truck, or vehicles that are suitable in size, capacity and number to meet project needs.
Vehicles used for these services shall reasonably accommodate the item capacity needed to fulfill the project, and shall be enclosed to protect items from damage and weather.
The Contractor’s personnel shall wear appropriate and professional looking attire. Athletic and athletic-style garments (i.e., jogging suits, sweat suits, running shorts, tee-shirts) shall not be worn by the Contractor’s employees. Steel toed shoes are recommended. Hard hats may be required in some areas.
The Contractor’s personnel shall conduct themselves in a professional manner while on VAPHS property. Personnel shall be courteous with VAPHS staff and visitors and comply with directions from VAPHS Police Service, Infection Prevention and the Contracting Officer’s Representative (COR).
VAPHS is a smoke-free workplace. Smoking, vaping or other tabaco use is not permitted.
VHA Healthcare Directives
1. The Contractor Shall adhere to the following VHA Directives and policies for COVID-19, Influenza Vaccination, and Tuberculosis Screening:
1. COVID-19: The Contractor shall comply with current VAPHS policy, which may include masking while on VAPHS property. As COVID policies may change based on current conditions, the COR shall provide guidance to the Contractor on current policy. If masking policy mandates the use of a certain types of masks (N-95, etc.) VAPHS shall provide these to the Contractor’s personnel.
1. Influenza Vaccination: VHA Directive 1192 requires all health care personnel (HCP) to participate in the seasonal influenza prevention program and outlines the key implementation steps. Health care personnel is defined in VHA Directive 1192 as all VA licensed and unlicensed, clinical and administrative, paid and unpaid, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, students, researchers, volunteers, and trainees who work in VHA locations. Masks are available at entrances throughout the medical center.
1. It is the responsibility of the contractor to ensure that all contract staff are compliant with the requirements outlined in VHA Directive 1192. The contractor shall maintain the following documentation:
3. Documentation of vaccination, e.g., signed record of immunization from a health care provider or pharmacy, or a copy of medical records documenting the vaccination
3. Completed Health Care Personnel Influenza Vaccination Form (Attachment B of VHA Directive 1192) if unwilling or unable to receive the vaccine.
3. The contractor is required to submit annual certification to the Contracting Officer’s Representative (COR) that all contract staff performing services at VA facilities are in compliance with VHA Directive 1192. . (VAPHS shall NOT require names or records of vaccination status)

1. Tuberculosis Screening: VHA Directive 2011-036, Safety and Health During Construction. Contractor shall certify annually to the COR(s) that all Contractor employees working at VAPHS facilities have a documented screening for latent Tuberculosis within the last 12 months. If screening for latent Tuberculosis is positive, clearance from a physician is required prior to working at VAPHS. Contractor employees that fail to meet this requirement will not be permitted to work at VAPHS facilities and may be asked to leave VAPHS property without notice until the requirement is met. (VAPHS shall NOT require names or records of screening status)
Pre-Invoice Review
1. The Contractor shall submit a list of services provided to the COR for review on the 5th of every month for the prior month. The Contractor shall not submit invoices until the COR has reviewed and approved the list of charges.
1. Services shall be listed by the type and number of services hours provided as follows: (project forms shall be provided by the COR at the start of projects)
2. Labor Type – supervisor, skilled laborer, general laborer.
2. Vehicle Usage – van, delivery van, box truck, twenty foot truck.
2. Name of project(s) supported – locations (bldg.., room #), dates, names and position title of personnel that performed the work.
Contracting Officer’s Representative The COR responsibilities include all responsibilities outlined in the COR delegation letter, as well as surveillance of services required under this Contract; determining the adequacy of Contractor performance according to the specifications of this Contract; acting as the Government’s representative; ensuring compliance with Contract requirements insofar as the work is concerned; and advising the CO of any contract issues. The COR cannot authorize deviations from the terms and conditions of this contract. In the event the Contractor deviates without written approval from the CO, such deviation shall be at the risk of the Contractor, and any cost related thereto shall be borne by the Contractor. The COR shall be designated at the time of Contract award and appointed in writing by the CO to assist in the discharge of CO responsibilities when the CO is unable to be directly involved with the Contract work. The CO and COR shall meet at least quarterly to review invoices and address any Contractor performance issues. The following is the list of designated CORs:

Contractor Representative The Contractor shall provide the name, address, telephone number, and email address of authorized Contractor representative(s) who have the binding authority to act on behalf of the Contractor on administrative and/or performance matters. The Contractor shall maintain this list and submit any changes to the COR.
Other Requirements
1. Contractor’s Responsibilities and Insurance Requirements: The Contractor shall obtain all necessary licenses and permits required to perform the work described in this performance work statement. The Contractor shall exercise all precautions necessary to protect persons and property from injury or damage.
1. The Contractor shall be responsible for any injury to their personnel, or others, and for damage to property that is a result of Contractor employee negligence.
1. The Contractor shall maintain personal liability, automobile liability, and property damage insurance, as prescribed by the laws of the state of Pennsylvania, and in accordance with VA Acquisition Regulation 852.228-71; Indemnification and Insurance. Evidence of coverage shall be provided to the COR prior to services being provided. Coverage shall not be changed or cancelled without thirty (30) calendar days prior written notice to the CO.
5. Safety Requirements: In order to protect VAPHS staff and visitors, the Contractor shall take all safety precautions as the COR determines are necessary. The COR shall notify the Contractor of any non-compliance with these policies and the required corrective actions. If the Contractor fails or refuses to comply, the CO may issue a stop-work order and hold the Contractor in default. Noncompliance may result in Contract termination.
6. The Contractor’s personnel are required to interrupt their work at any time so as not to interfere with the normal functioning of VAPHS facilities, including utility services, fire protection systems, and passage of patients, personnel and equipment. These interruptions shall only be occasional. Any interruptions considered to be excessive should be brought to the attention of the COR.
7. The Contractor shall not dispose of hazardous waste or materials at any VAPHS locations except as specified by the COR.
Security Requirements
1. The COR shall provide the Contractor with VA identification badges that shall be worn while on VAPHS property.
The Contractor shall not have access to the VA network or any VA sensitive information.
Contractor personnel performing services at VAPHS shall complete VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access to or Use to VA Sensitive Information annually.
Training shall be completed by using the VA’s Talent Management System (TMS) @ https://www.tms.va.gov, using the self-enrollment option, or by completing the hard copy version. Certificates of completion shall be submitted to the COR
