Combined solicitation/synopsis NOTICE TO CONTRACTOR: RFP IHS1460974
This is a COMBINED SOLICITATION/SYNOPSIS for commercial items/service prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This solicitation has been prepared with the intent to award a Firm Fixed Price (FFP) contract, utilizing procedures under FAR Part 12. The proposed contract action is to provide Licensed Practical Nurses (LPN), Certified Medical Assistants (CMA), Certified Surgical Technicians (CST) and Certified Nursing Assistants (CNA) services for the Phoenix Indian Medical Center (PIMC), Indian Health Service (IHS), Phoenix, Arizona. This announcement constitutes the only solicitation. The Request for Proposal (RFP) number IHS1460974. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-77 and the Health and Human Services Acquisition Regulation (HHSAR). It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The North American Industry Classification System (NAICS) Code for this acquisition is 621399 with size standard of $8MIL; Product Service Code (PSC) is Q401 – Medical Nursing

The Government will award a firm, fixed price contract resulting from this combined synopsis/solicitation to the responsive and responsible offeror whose conforming response is determined to provide the best value to the Government, price and other factors considered. The Government will only consider firm fixed-price quotations.
Evaluation Factors/Criteria:
(a) The major evaluation factors for this solicitation are technical, (which include how well the vendor has conformed to the critical terms contained in the SOW), relevant past performance and cost/price factors. Although technical factors are of paramount consideration in the award of the contract, cost/price is also important to the overall contract award decision. All evaluation factors other than cost or price, when combined, are significantly more important than cost or price. In any case, the Government reserves the right to make award(s) to that offeror whose proposal provides the best overall value to the Government.
Offerors are advised that award will be made to that offeror whose proposal provides the combination of features that offers the best or greatest overall value to the Government. The Government is more concerned with obtaining performance capability superiority rather than lowest overall cost. However, the Government will not make an award at a significantly higher overall cost to the Government to achieve only slightly superior performance. Overall cost to the Government may become the ultimate factor for award of a contract as proposals become more equal based on other factors.
The evaluation will be based on the demonstrated capabilities of the prospective Contractors in relation to the needs of the project as set forth in the RFQ. The merits of each proposal will be evaluated carefully. Each proposal must document the feasibility of successful implementation of requirements of the RFP. Offerors must submit information sufficient to evaluate their quotes based on the detailed criteria above.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
Offeror to be considered responsible shall have an updated data and information in System for Award Management (SAM), FAR provision 52.212-3 Offeror Representations and Certifications-Commercial Items (APR 2012). The provision can be submitted at https://www.sam.gov. Contractor must be registered to the System for Award Management (SAM) prior to award. A nine digit Data Universal Numbering System (DUNS) number provided by Dun and Bradstreet is required to register.
Electronic submission of Proposals: Offers shall be submitted electronically via email to dale.clark@ihs.gov. Commercial rates sheets, quotes, past performance and technical data should also be attached.
QUANTITIES - The estimates specified in the invitation are derived from the best estimates available at this time and are given for information to bidders and for the purpose of evaluating bids and not as an indication of actual requirements. The quantities to be ordered will depend entirely upon requirements and no firm obligation is given or intended. The Contractor shall be required to furnish any or all services and/or supplies in accordance with the specifications and at the unit prices indicated therein, when such services and/or supplies are authorized by the proper ordering official. It is understood and agreed that the Indian Health Service will order from the Contractor all of such services and/or supplies called for in this contract, as there may be need for in the judgment of the proper official, during the contract period.
CLAUSES AND PROVISIONS - This solicitation incorporates the following FAR and HHSAR Clauses and Provisions as well as applicable GSA/VA FSS contract clauses.

52.217-8 Option to Extend Services (Nov 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 calendar days.

(a) The Government may extend the term of this contract by written notice to the Contractor within 10 calendar days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 calendar days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years and 6 months.
