About this file
This document is an Attachment A: Capabilities Questionnaire for a laboratory services contract. The questionnaire requires potential contractors to verify their qualifications and capabilities across multiple critical areas, including laboratory certification, licensing, experience, and potential subcontractor arrangements. Key verification requirements include confirming the laboratory is fully licensed and accredited by organizations like College of American Pathologists (CAP) and The Joint Commission (JTC), meeting Clinical Laboratory Improvement Act of 1988 requirements, having a certified Pathologist/Bio-analyst as Laboratory Director, possessing at least three years of laboratory services experience, and having all necessary CLIA certifications. If a subcontractor will be utilized, the questionnaire mandates detailed information about the subcontractor, including their credentials, business details, and a letter agreeing to perform tests not completed by the primary contractor.
