About this file
This document is a Required Affidavit form from the Commonwealth of Kentucky, specifically related to KRS 45A.110 and 45A.115, which governs bidders, offerors, and contractors participating in state contracts. The affidavit is a mandatory document that must be completed by an authorized representative of a contracting party when seeking to be awarded or renew a contract. The form is valid for one year from the date of execution and requires the contractor to attest that they have not knowingly violated campaign finance laws of Kentucky.
The affidavit requires the bidder or offeror to swear under penalty of perjury that they are aware of and will comply with campaign finance laws. The form includes spaces for the contractor's signature, printed name, title, date, company name, address, and optional vendor code. It also includes a notary section where a notary public will confirm the attestation, recording the date, state, county, and the notary's commission expiration. The Commonwealth retains the right to request an updated affidavit at any time, and "knowingly" is defined as being aware or should have been aware of any potential violations.
