About this file
This is a quality assurance inspection form for forested plot data collection under the Coastal Alaska Inventory program, updated January 2, 2026. The form functions as a standardized checklist for verifying the accuracy and completeness of electronic plot folder data before final submission.
The form covers eight major inspection categories with specific verification requirements for each: Plot Card (legibility, correct completion per Appendix N.6, and presence of access sketch maps); Plot Data (header information verification, narrative field accuracy, and discrepancies between plot narratives and digital data); GPS Data (survey-grade coordinate collection on accessible forest land subplots with explanations for missing coordinates); Site Tree Data (appropriate site tree selection, hierarchical criteria compliance, and documentation of alternative scenarios); Vegetation Data (species code clarification); Tree Data (reconcile codes, diameter checks, damage codes, mortality year alignment); Plot Printout (note clarity and validation explanations); Ownership (NOMS screen records, plot center owner designation, and age tree collection requirements); Condition Data (forest type, stand size class, regeneration status consistency, stand age updates, fuelbed type verification, and canopy cover documentation); and Boundary Data (boundary change documentation and boundary viewer diagram verification). The form includes dedicated sections for items requiring correction with QA notes and signature lines, as well as sections for QA notes where corrections are not required, allowing for comprehensive documentation of the inspection process and communication between field crews and quality assurance personnel.

C ar d All versions are legible and clear ☐ Filled out correctly per instructions in Appendix N.6 ☐ Plot Access sketch map is present if there has been a significant change since the last visit, if the previous sketch map is of poor quality, or if a cover check was completed.

Verify the plot card header information matches the plot data in the MIDAS file. ☐
Verify PLOT NARRATIVE and the PLOT ACCESS DESCRIPTION are in the correct data fields. ☐
Check for discrepancies between the PLOT NARRATIVE, the digital plot data, and the maps on the plot card.
PLOT ACCESS DESCRIPTION and PLOT NARRATIVE are completed following protocol in field guide.
If any MAPER = Y: plot, condition, subplot, boundary, and change matrix notes are appropriate;
conditions, boundaries, and change matrix are corrected/reconciled.

D at a Survey Grade GPS coordinates are collected on all subplots that have an accessible forest land class present on the subplot/macroplot. If coordinates are not collected on a subplot, appropriate GPS NOTES explain why.
If present, any GPS warnings related to PC coordinates are explained by the field crew (i.e. “Plot was mislocated by 500’ by the previous crew, current GPS coordinates correct.”)

SITE TREE NOTES only pertain to that site tree record. Site Index notes pertaining to the entire plot or a condition class are recorded in PLOT NOTES including when no site trees are collected for a plot or condition.
Old site trees not re-used have a SITE TREE NOTE explaining why (e.g. wrong species, site index too low, etc.).
All current site trees adhere to the hierarchical site tree selection criteria. ☐ If a site tree is collected as a ‘last resort’ there are no additional site trees collected for that condition class. QUESTIONABLE SITE TREE = 0 with a SITE TREE NOTE explaining why.
If the required number of site trees are not collected and only one site tree is collected because no others are available, and the site tree is good, QUESTIONABLE SITE TREE = 1. Make sure a PLOT NOTE explains the situation.
If there are no site trees recorded for a condition there is a PLOT NOTE describing the site’s topography, moisture regime, substrate material, and general tree growth.
V eg
D at a Unknown or generic species codes (forbs and grasses) have a note describing the plant characteristics to allow differentiation from other species on plot.

Review all TREE RECONCILE codes to verify they make sense ☐ If a TREE NOTE alludes to DBH being updated, moved, or old nail missing, verify DIAMETER
CHECK = 2.
If DIAMETER CHECK CODE = 2 make sure there is an appropriate TREE NOTE. ☐
Review all DAMAGE codes to verify they make sense. ☐
Review all TREE NOTES. Are all TREE NOTES consistent with the recorded tree data? Do any TREE NOTES allude to an error in the recorded tree data? Are any notes ambiguous or unclear to someone that is not on plot?
When CAUSE OF DEATH = 70, there is a TREE NOTE describing it. “Unknown” is an acceptable note, but if the note alludes to a cause of death, it should be coded as such.
Tree MORTALITY YEAR matches Condition DISTURBANCE/TREATMENT YEAR when applicable. ☐
Pl ot
P ri nt ou t Review all notes to make sure they are clear, support the data, explain anomalies in the data and are recorded for the correct data item.
Review all validations and make sure all explanations address the validation. Check to determine if there are errors in the data associated with the validation.
O w ne rs hi p All owners on plot have a record in the Ownership NOMS screen in MIDAS. ☐
One owner is checked as owner at plot center, and it is the correct owner. ☐
Three age trees are collected that are within the current stand size class for each condition. If unable to obtain 3 age trees that meet these criteria, an explanation is provided in PLOT NOTES.

If FOREST TYPE, STAND SIZE CLASS, REGENERATION STATUS, or TREE DENSITY data items are different than the previous data due to previous crew error or not otherwise explained in the data, make sure there is a CONDITION CLASS NOTE explaining if the change is real change or previous crew error.
Review FOREST LAND CONDITION STATUS CHANGE code for accuracy. ☐
If a stand is relatively undisturbed and PREVIOUS STAND AGE seems correct, STAND AGE is updated by adding the number of years since the previous inventory and AGE BASIS CODE = 40. If not, then the appropriate AGE BASIS CODE is used.
Verify the recorded FUELBED TYPE is reasonable for the condition. ☐
If LCCV ≠ LMCC, there is a CONDITION CLASS NOTE explaining the situation, unless a Disturbance or Treatment is recorded.
Verify recorded COVER CLASS is consistent with recorded LIVE CANOPY COVER. ☐
B ou nd ar y D at a If there is a change to boundary data from the last inventory, verify that boundaries have been recorded correctly according to instructions in the Condition Reconcile Table, with notes in the correct location. ☐
Check BOUNDARY CHANGE CODE and verify correctly coded. ☐ Review the Boundary Viewer diagram and cross check with plot card mapping to verify condition areas are correct and mapping rules followed. ☐
