About this file
This document contains a Bureau of Land Management Herbicide Treatment Record template. The template collects key details about herbicide application projects, including applicator information, treatment location, weather conditions, herbicide and adjuvant specifications, treatment area characteristics, species treated, and application metrics.
Relevant details include treatment start and finish dates and times, location coordinates, applicator name and license number, temperature, wind speed and direction, humidity, soil moisture, distance to water, equipment type, diluent, township-range-section, calibration rate, coverage pattern, trade names and EPA registration numbers of herbicides and adjuvants, mix rates and units of measure, species codes, phenology, treated area type, cover percentage, total mix volume applied, and notes. This level of record keeping appears to be required for herbicide application projects on federal lands managed by the Bureau of Land Management.
The related federal contract opportunity is for Noxious and Invasive Plant Treatment Services on an indefinite-delivery/indefinite-quantity basis with the Department of the Interior Bureau of Land Management offices in Oregon and Washington.

2. Treatment Start and Date: The date MM/DD/YYYY, Hour: Minutes, that application of herbicde began , e.g. 04/12/2020 , 13:00
6. Location Accuracy: Enter the known level of accuracy in feet based on equipment tolerances.
16. Coverage Pattern: Choose the one best descriptor for the application coverage type.
20. Manufacturer: Enter the name of the company that produces the herbicide.
21. EPA registration No. : Enter the Environmental Protection Agency registration number.
22. Mix Rate: Enter the quantity of herbicide that is used in the tank mix.
23. Unit of Measure: Enter the units of product over the units of diluent e.g. fluid oz. / gallons.
24. Trade name: Enter the brand name of the adjuvant(s) used.
25. Enter the quantity of adjuvant that is used in the tank mix.
26. Unit of Measure: Enter the units of product over the units of diluent e.g. fluid oz. / gallons.
