About this file
This document contains a past performance questionnaire template for a federal solicitation. The solicitation is seeking offers for therapeutic medical physicist and medical dosimetrist support services at Travis Air Force Base, California. Offerors are asked to have their references complete the questionnaire by January 19, 2024 at 11:00 AM Pacific Time and return it directly to the contracting specialist. The questionnaire collects information on the offeror's performance on relevant prior contracts over the past three years, including quality of services provided, management efforts, flexibility in addressing issues, ability to recruit and retain qualified personnel, and timeliness of credentialing packages. It also asks if the offeror has been rated in CPARS or received any cure or show cause notices on prior contracts. The completed questionnaires will become part of the evaluation record for selecting the awardee of this full and open competition solicitation issued by the Department of the Air Force Air Mobility Command.
