Transmittal Letter to Accompany Past Performance Questionnaire

MEMORANDUM FOR: [Government Customer]

SUBJECT: Present/Past Performance Questionnaire for [Past Performance Information Submission Selection]

1. We are currently responding to the DARPA Technical & Analytical Support Services Request for Proposals (RFP) HR001123R0001 as a [prime contractor or subcontractor to Offeror]. This RFP specifically requires that we, as an Offeror, send a Past Performance Questionnaire to our identified points of contact (POCs) and track its completion and return.

2. We identified the subject contract / task order as relevant to this acquisition and you as our POC. As such, please take a few moments of your time to fill out the attached questionnaire and return it directly to the POC indicated below. The information contained in the completed questionnaire is considered source selection sensitive and cannot be released to us, the Offeror. If you have any questions about the acquisition or the attached questionnaire, your questions must be directed back to the below POC.

3. Please email the completed questionnaire directly to the Government no later than 10:00 AM Eastern on XXday, March XX, 2023, at:

Indicates the contractor is deficient and demonstrates very little understanding and ability to perform the goals/objectives of the contract. Such deficiencies are expected to be very difficult or impossible to correct.
Indicates contractor’s performance does meet contractual requirements, however, the area of evaluation experienced serious problems that were difficult to correct. Corrective action was either not identified, appeared only marginally effective, or was not fully implemented.
Indicates the contractor’s performance meets contractual requirements and the goals/objectives of the SOW/PWS. The area of evaluation experienced some minor problems for which: either, 1) corrective action was taken and appears to be satisfactory, or 2) corrective action was identified that will probably correct the problems.
