The template allows subcontractors/teaming partners to give consent for the Government to share their past and present performance information with prime contractors during source selection evaluations. The letter must be completed by all subcontractors/teaming partners identified in the proposal and submitted as part of the Past Performance Volume. The template includes fields for company information, authorized signature, and must be addressed to the Contracting Officer Jon Prihoda. No consent letters are required if a proposal does not include any subcontractors or teaming partners.

PAST PERFORMANCE CONSENT LETTER

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF

PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR

[Past performance information concerning key personnel, subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor‘s or teaming partner‘s consent.

Because a prime contractor is a private party, the Government will need that consent before disclosing program manager, subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume. If there are no subcontractors or teaming partners as part of your team, then no consent letters are required with your proposal.]
