About this file

The document is an Attachment 4 Cover Form for an Invitation to Negotiate (ITN) issued by the state of Idaho. The cover form must be completed, signed, and submitted with the proposal. The ITN is for Pre-Employment Transition Services Activities for the Idaho Division of Vocational Rehabilitation and covers Ada County. The form requires the offeror to provide information about their company status, tax ID, DUNS number, and willingness to comply with the ITN requirements. The offeror must affirm they have not used improper means to secure the contract, are not currently suspended or debarred, and the proposal will remain firm for 90 days. The contract term is not specified.

The cover form does not include details on pricing, set asides, incumbents, funding sources, or other salient commercial terms. The focus is on collecting administrative information about the offeror to determine if they are a qualified and responsible bidder.
