HAZMAT USAGE LOG - FOR CONTRACTORS
All HAZMAT containing VOCs or HAPs shall be reported using this HAZMAT Usage Log to comply with Division Newport's Air Emissions Cap Permit, No.1292 and associated regulations.
This log is due monthly, no later than the 10th day of the following month, or promptly at the completion of the project, whichever comes first.
I certify that: (1) All HAZMAT Safety data sheets have been reviewed and approved for use by NUWC Division Newport's Environmental and Safety Branches prior to bringing HAZMAT on the installation. (2) All architectural and industrial coatings (including, but not limited to: paints, primers, enamels, and sealants) shall comply with RIDEM Air Pollution Control Regulation (APCR) No. 33 and the limits listed at §33.7(B). (3) All adhesives must comply with RIDEM's APCR No. 44 and the emission limits listed at §44.7.1 and §44.7.2. (4) Solvents associated with the application or removal of adhesives and sealants must comply with the VOC and Vapor Pressure limits in, APCR No. §44.7(C) and §44.7(D).

1b. HAZMAT Name 1c. Product Purpose NIIN: Enter the NIIN (national item identification number) of the ammunition item or part number 1d. Manufacturer Name Lot Number/Serial Number: Enter the lot number or serial number of the ammunition item. The lot number is the unique code number assigned to a quantity or batch of a single ammunition end item. The serial number is the unique code assigned to the all-up round or major ammunition end item (rocket, guided missile, torpedo, underwater mine, etc.).
1e.VOC Content (lbs./gal) Ownership Code: Provides the means for segmenting and identifying the service or agency having established ownership of the asset Gallons Used Grid: Enter the grid for the location inventoried. If no grid exists for the inventoried storage location, enter “N/A” in this field.
