SUBJECT: Request for Past Performance Information for Solicitation

W9127Q-24-R-0020, Technical Instruction Services for a “Commercial Vehicle Interdiction” Course

1. The USPFO-MS Purchasing & Contracting Office is in the process of selecting a contractor to provide Technical Instruction Services for the subject course sponsored by the Regional Counterdrug Training Academy (RCTA) in Meridian, Mississippi. All offerors submitting proposals in response to this solicitation are required to provide references, to include points of contact, for similar services in order to assist the government in evaluating past performance.

2. Please complete the attached Past Performance Questionnaire (PPQ) and submit it directly to the individual identified in paragraph 4 below. The PPQ should completed by the person(s) within your firm/agency who are most familiar with the offeror’s performance on the identified contract(s). Evaluations should reflect an honest, straightforward, and objective evaluation of the offeror’s performance based on objective facts.

3. As your knowledge is crucial to our evaluation of the offeror’s past performance, we request you provide responses to all questions. A “Not Observed” answer may be appropriate when no evidence is available to you in a particular area. Indicate, based on the definitions provided in the questionnaire, the offeror’s performance on the contract(s) identified. Please provide narratives to support your ratings, especially any that are marked “Exceptional,” “Marginal,” or “Unsatisfactory.” Hand written responses, printed clearly, are sufficient. Any other comments you wish to provide are welcomed.

4. Your attention to this matter would be greatly appreciated. Please submit the PPQ directly to Brian Fortune via email brian.p.fortune.mil@army.mil

PPQ submissions are due no later than 15 August 24. It may be necessary to contact you to discuss questionnaire responses. As this questionnaire relates to an ongoing source selection, once completed it should be handled as Source Selection Sensitive

Information in accordance with FAR 3.104. We will safeguard the information, to include not releasing the names of any respondents outside the Source Selection

Evaluation Board (SSEB).
