About this file
This is a Technical Capability and Experience Questionnaire template for prime contractors responding to USDA APHIS's solicitation for harvest of ash trees infested with Emerald Ash Borer (EAB). The questionnaire requires contractors to demonstrate their capacity to execute three separate harvesting jobs involving tree removal in Wisconsin, transportation to Michigan facilities in Howell and Brighton, and unloading operations. Respondents must detail crew numbers, crew sizes, equipment resources, and provide a comprehensive schedule addressing tree harvesting activities, material transportation logistics, and unloading procedures for each of the three jobs, with work commencing March 1, 2026.
The questionnaire mandates submission of current International Society of Arboriculture (ISA) certification for the proposed arborist or State Master Logger Certification from any state, with subcontractor certifications requiring commitment letters. Additionally, contractors must provide three references from relevant forest tree harvesting operations completed within the past five years, including agency/firm name, contact information, contract number, award amount, dates, work role classification, descriptions of self-performed work demonstrating relevance to the requirement, and disclosure of any quality, timeliness, or customer service concerns with corresponding corrective actions taken. This questionnaire is a critical evaluation component for assessing contractor technical capacity and past performance in commercial timber harvesting operations.
