About this file

This document is a dental laboratory services contract for the Fort Yuma Health Center (FYHC) Dental Program. The contract specifies requirements for fabricating dental prosthetics, including complete dentures, removable partial dentures, and fixed prosthodontics like crowns and bridges. Key requirements include using specific materials like Lucitone 199 acrylic resin and Vitallium metal bases, providing a 1-year warranty on all prosthetic cases, and offering remakes at no charge if the restoration does not meet clinical standards.

Patient selection criteria are detailed, prioritizing CHS-Direct eligible patients within the service unit boundaries, with preference given to medically compromised patients. The dental laboratory must provide monthly prosthetic unit fabrication services, use new materials in original sealed containers, and ship prosthetics via expedited shipping (typically overnight) with tracking. The contract allows for digital scanning technology, requires dental labs to be evaluated ongoing, and permits labs to be added, dropped, or changed at any time. Shipping costs, temperature control during transit, and specific performance monitoring by the point of contact are also comprehensively outlined.
