STATEMENT OF WORK

FOR

WELL 5 INSPECTION AND REPAIRS

9 July 2024

1. DESCRIPTION OF SERVICES. The contractor shall provide all labor, equipment, tools, materials, supplies, and other items and services necessary to perform the inspection of Well 5 vertical turbine pump within the base drinking water plant, Building 90355 at Hurlburt Field, Florida. All work shall be accomplished IAW all State and Federal Laws and the this Statement of Work (SOW).

1.1. SCOPE OF WORK.

1.1.1. The Contractor shall set up crane and remove the 50 horse power well motor, well pump discharge assembly, 125 feet of column pipe, line shafts and existing drawdown air line.

Video 714 feet of 12 inch diameter well casing and provide digital copy. Disassemble and inspect entire pumping system. Prepare quote for needed repairs and recommend rehabilitation procedures if necessary. Repairs if needed should be completed prior to re-assemble amd install of all equipment to include drawdown line if damaged. Air line will be attached to the column pipe at the point where the last threaded column pipe joins the impeller bowl assembly; secured at least every ten (10’) foot interval utilizing stainless steel banding from this point, upward, to the well pump discharge assembly in a manner that will not crush or cut the air line and attached to the bottom of the existing air gauge with an industry standard fitting.The Contractor shall be responsible for disconnecting all elctrical and mechanical connections prior to removal and and re-connecting prior to start up.

1.1.2. The Contractor shall disinfect the well in accordance with Section 1 through 4, and Section 5.2 of American Water Works Association (AWWA) STD 654-13. Sampling shall be in accordance with Florida Administrative Code (F.A.C) 62-555.315(6)(c)(1-3).

1.1.3 The government will perform a vibration test on the well motor prior to the start of the job and record all readings in the presence of the Contractor. The government will perform a vibration test after the completion of job. Readings shall be within 10% of initial readings. Cost to return pumping assembly to established readings will be at the Contractors’ expense.

1.1.4. The Contractor shall provide an on-site supervisor who may or may not be designated as the contract manager, who will be responsible for the work and work crew during the removal process, air line installation, re-install process and field testing of the well. The on-site supervisor shall coordinate the day to day work and any issues with the 1 SOCES Base Water Manager, or in his absence his designated representative.

1.2. QUALITY ASSURANCE

1.2.1. The Contractor shall assume responsibility for the satisfactory installation and operation of the entire pumping system. The motor is currently fully operational/balanced, and the Contractor shall return the motor back to 100% fully operational/balanced condition. All costs associated with bringing the motor back to 100% fully operational/balanced condition, or better, is the sole responsibility of the Contractor. It is the Contractors responsibility to monitor the work to ensure the well pumping system is brought back into full operation within the ten (10) business day period of performance.

1.2.1.1. All equipment parts shall be properly protected so that no damage or deterioration will occur during a delay from the time of shipment until installation is completed and the well pumping system is ready for installation/testing/operation.

1.2.1.2. All equipment and parts must be properly protected against any damage during shipment. Store equipment in accordance with the manufacturer's instruction.

1.2.1.3. Factory assembled parts and components shall not be dismantled for shipment unless permission is received in writing from 1SOCES/CEOIU.

1.2.2. The Contractor shall have the well tested for total coliform IAW AWWA STD 654-13 and F.A.C. 62-555.315(6)(c). The wasting of the disinfectant, sample collection, and transportation to the lab will be provided by the government’s representative. All costs associated with this bacteriological testing and analysis process are the Contractor’s responsibility until two consecutive samples show the absence of total coliform , as referenced in Section 1.1.2, of this SOW. If the well tests positive for total coliform for consecutive samples, the Contractor shall disenfect the well IAW AWWA STD 654-13 and sampling shall be in accordance with Florida Administrative Code (F.A.C) 62- 555.315(6)(c)(1-3).

1.3. The work site shall be returned to the condition prior to start of work to include but not limited to the facility and grounds.

1.4. All materials that can be recycled for the duration of the contract will be brought to the Base Recycling Center by the Contractor.

1.5. Any damges to any equipment on the job site shall be the Contractor’s responsibility to repair at no cost to the Government.

1.6. The contract manager shall respond to written or telephonic concerns of the Government Contracting Officer, Contract Administrartor, and/or Base Water Manager within four (4) hours from the contact either by email or a phone call.

1.7. The equipment provided under this SOW shall be warranted for a period of three (3) years for parts and labor.

2. GOVERNMENT FURNISHED PROPERTY AND SERVICES.

2.1. General Information. No Government property, equipment, vehicles, supplies or labor will be provided for this requirement. The Contractor shall furnish all required equipment, vehicles, supplies and labor required to fully perform the requirement listed in this SOW.

2.2. Government Furnished Utilities. No government furnished utilities will be required or provided.

2.3. Government Furnished Facilities. No government furnished facilities will be required or provided.

3.0. GENERAL INFORMATION.

3.1. Contract Manager. The Contractor shall provide a contract manager who shall be responsible for the performance of the work.

3.2. Contract Employees: The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the contracting officer as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

3.2.1. All contract employees shall be properly trained and certified (if required) to perform the required tasks

3.2.2. Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees.

3.2.3. To be considered technically acceptable for consideration for the work listed in this SOW, the Contractor shall have at least ten (10) years experience in the work described in Section 1 of this SOW.

3.3. HOURS OF OPERATION The contractor shall perform the services required under this contract during the base hours of operation: 0600 to 1500 Monday through Friday. For emergency service calls the contractor shall respond 24 hours a day, 7 days a week, including weekends, nights, and U.S. Government holidays. The Contractor may find it necessary todeviate from the normal base hours of operation (i.e. working on Saturday or Sunday) due to weather conditions or unforeseen circumstances, to ensure timely completion of work under this PWS at no additional cost to the government. Work performed outside of the normal base hours will have to be approved 24 hours in advance by the AGR.

3.4. Re-performance of Service. When service performed does not conform to contract requirements, the Contractor shall perform or re-perform the service in conformity with contract specifications at no additional cost to the government.

3.5. Government Remedies. The contracting officer shall follow FAR 52.212-4(a), Inspection and Acceptance Clause, for Contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

3.6. Federal Holidays. The following dates are considered federal holidays and no preventative maintenance shall be scheduled during these dates; any calls during these dates will be considered emergencies. If performance is required for these holidays, it shall be approved and scheduled in advance with the AGR.

3.7. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base. Delays caused by the Government or otherwise covered in this section shall not countagainst the contractor.

3.8. Contractor/Employee Base Pass and Identification, Special Clearances, and Vehicle Passes.

3.8.1. The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation.

3.8.2. The Contractor shall be required to complete the Contractor Employee Listing (provided at the pre-performance conference) in order for employees to gain access to the installation.

3.8.2.1. The Contractor Employee List. This form shall list every employee requiring access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals who require escort authority by including the word “escort” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations please use building numbers or areas (i.e. Bldg 90618 or Red Horse, AMMO Area, etc.). Once completed, email the list to the Contracting Officer in EXCEL or WORD format. The Contracting Officer will then prepare and attach a cover letter, and forward the Employee List to the Visitor Control Center.

3.8.3. During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

3.8.4. When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management, as applicable.

3.8.5. Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees are returned to the issuing office.

3.8.6. Failure to comply with these requirements may result in withholding of final payment.

3.9. Hazardous Materials Use. The contractor shall obtain approval from the Hurlburt HAZMART for any chemicals used during the term of this contract. Any products that meet the criteria of “Hazardous Waste” must be bar-coded and tracked until permanently removed from government property. The inventory will be performed prior to commencement of work. Criteria for identifying hazardous waste are contained in Subpart C of 40 CFR, Part 261.

3.9.1 Spill Response. The contractor will be briefed on Hurlburt Field spill response procedures at the pre-performance conference. The contractor is responsible to report and promptly cleanup all spills in a manner consistent with current environmental regulations.

In the event that it is necessary to utilize government material, equipment, or personnel to clean up a contractor caused spill, the contractor shall be required to reimburse the government for all associated costs.

3.10 Hazardous Material/Waste Management. The contractor will be briefed on Hurlburt Field Hazardous Material/Waste Management Plan at the pre-performance conference.

3.10.1 Hazardous Material Handling. The contractor shall obtain approval from the base HAZMART section prior to purchasing, handling, using, and storing any chemicals, solvents, lubricants, and other products that require Material Safety Data Sheets (MSDS).

The contractor shall identify these materials and products on Air Force Form 3952, Chemical/Hazardous Material Authorization Request, Process Identification Form, and Shop Disposal Form. The contractor shall provide one copy of the MSDS for each item to HAZMART section for review prior to any chemicals being brought onto Hurlburt Field.

The Contractor shall maintain one copy of the MSDS for each hazardous material line item used within the work center.

3.10.2 The contractor shall not use products that are, or contain, Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals. Any hazardous material containing one of these banned substances will not be allowed on base.

3.11 Traffic Laws. Contractor personnel shall comply with all base regulations, and are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, except hands-free phones, is prohibited while driving on Hurlburt Field.

3.12 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned or privately owned vehicle while on Hurlburt Field.

3.13 Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information.

3.14 Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

3.15 Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, rain gear, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.

3.15.1 Licenses and Permits. The contractor shall, without additional expense to the government, obtain any necessary licenses and permits, and comply with any Federal, State, and local laws, codes, and regulations applicable to the PWS. The contractor shall provide proof of such applicable licenses and permits upon request by the government.

The contractor shall submit any applicable certifications obtained, i.e. ISO-9000, Quality Excellence awards etc.

3.15.2 The government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

4.0 SAFETY REQUIREMENTS

4.1 The contractor shall be required to furnish signs, which will be placed when work is being performed, stating “Hoist being serviced – DO NOT USE” and “Overhead Work”.

The signs will be removed immediately after overhead work is accomplished. The contractor shall also establish “clear zones” by barricading or roping off the area prior to performing hazardous/overhead work as referenced in EM 385-1-1, Safety and Health Requirements Manual. If a building has more than one hoist, only one hoist will be serviced at a time, unless approved by the COR prior to the commencement of the repair(s).

4.2 The contractor shall protect government property to prevent damage during period of time the property is under the control or in possession of the contractor.

4.3 Mishaps. In the event of an accident or mishap, the contractor shall take reasonable and prudent action to establish control of the accident or mishap scene, prevent further damage to persons or property, and preserve evidence until released by the COR.

4.3.1 The contractor shall record and report promptly (within 1 hour) to the COR, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

4.3.2 If the Government conducts an investigation of an accident or mishap, the contractor shall cooperate fully and assist the government personnel in conducting such investigation until the investigation is completed

4.4 Contractor Furnished Safety Items. The contractor shall furnish everything required to perform work identified in this contract according to all its terms. The contractor should be aware high reach lifts shall be provided by the contractor in the course of this contract in order to provide safe access to units up to 50 feet above finished floors.

4.5 Smoking. Smoking will be permitted in authorized areas only.
