About this file

This document is a Past Performance Questionnaire (PPQ) used to evaluate a contractor's performance across multiple dimensions. The form provides a comprehensive assessment framework with 30 performance categories divided into four main sections: Quality of Workmanship, Effectiveness of Management/Business Relations, Timeliness of Performance, Compliance with Labor Standards, and Compliance with Safety Standards. Ratings are binary - either Acceptable (A) or Unacceptable (U) - and cover areas such as quality control, resource management, schedule adherence, documentation submission, and safety implementation. The final section includes a general comment area where the evaluator can indicate their likelihood of awarding another contract to the contractor, with options ranging from "definitely would" to "definitely would not". The completed questionnaire must be emailed to specific Air Force email addresses by 20 January 2026 at 10:00 am AZOST, with explicit instructions not to return the document to the contractor.
