Each criterion uses a six-point scale: Excellent (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N). The form concludes with space for narrative comments addressing strengths, weaknesses, and performance risk factors. Clients are encouraged to submit completed questionnaires directly to offerors for inclusion in proposals, with the government retaining the right to verify all information provided.
