About this file

This document is a Past Performance Questionnaire to be filled out by a reference for an offeror on a Request for Quote (RFQ) issued by the Department of Homeland Security (DHS), Federal Emergency Management Agency (FEMA), for Level II Armed Security Guard Services in support of disaster response and recovery operations in Missouri.

The questionnaire requests the reference to provide an evaluation of the offeror's performance on a previous contract in the areas of staffing capacity, customer satisfaction, and responsiveness and attentiveness. The reference is asked to rate the offeror's performance as superior, good, satisfactory, marginal, unsatisfactory, or neutral, and provide any supplemental comments. This feedback will be used to evaluate the offeror's past performance for the current RFQ 70FBR724Q00000016, which is set aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB) with preference given to local firms in the affected areas of Missouri.
