About this file
This document is a Past Performance Questionnaire template to be used by the government to evaluate a contractor's past performance on a prior relevant contract. The questionnaire is divided into several sections to be completed by both the offeror and the government assessor. The assessor will provide an overall performance rating as well as ratings in the areas of technical performance, schedule performance, management, and cost control. The questionnaire covers key details such as the solicitation number, contract information, and the assessor's involvement. The evaluation criteria and definitions are provided to guide the assessor's ratings. This questionnaire template is being used in support of the federal contract opportunity for "Motor Generator Support Services" under Solicitation Number 1305M224Q0034, issued by the Department of Commerce's National Oceanic and Atmospheric Administration.
