About this file

This document is a Past Performance Questionnaire to be completed by a government customer regarding a contractor's past performance on a previous federal contract. The questionnaire covers various aspects of the contractor's performance, including technical quality, program management, transition/phase-in, employee retention, small business participation, and cost performance. The customer is asked to rate the contractor's performance on a scale of Satisfactory, Marginal, or Unsatisfactory, and to provide narrative explanations for any Marginal or Unsatisfactory ratings. The customer is also asked whether the contract was terminated, and to provide an overall assessment of the contractor's performance and whether they would have any reservations about using the contractor again. The questionnaire is part of a source selection process for a federal contract opportunity identified as the "West Point DPW MATOC" solicitation, which is being conducted by the Department of the Army Materiel Command Mission and Installation Contracting Command (MICC) Fort Eustis.
