About this file

This is a Vendor Self-Certification form for federal contract opportunity 47QSSC26Q0176, requiring vendors to attest that all technicians and installers meet specified safety and operational credentials for off-the-road (OTR) tire service work.

Vendors must certify that technicians possess TIA (Tire Industry Association) certification in either Earthmover Tire Service (ETS) or Commercial Tire Service (CTS), demonstrating specialized training in large OTR tire and multi-piece rim assembly hazards. Additionally, technicians must comply with OSHA 29 CFR 1910.177 standards for servicing multi-piece and single-piece rim wheels. Service providers must verify the credentials of personnel operating hydraulic boom cranes used on tire service trucks. Before work begins, vendors must supply or adhere to manufacturer torque and pressure specifications for the specific crane model, implement on-site safety protocols requiring mandatory remote inflation cages or restraining devices, and maintain Lock-Out/Tag-Out (LOTO) programs to ensure proper crane blocking and de-energization. Post-award, all vendor employees visiting the base must complete DIBIDS forms, provide two forms of identification, and undergo background checks through the Visitor Control Center. The form requires submission of company name, Unique Entity ID (UEI), socio-economic status, point of contact information, and quoter's signature with the quote submission.

Attachment 2 Vendor Self-Certiication

I certify that all technicians and installers will meet the following requirements (please provide credentials with quote submission):

1. Technician Safety Certiications

• TIA Certiication (Tire Industry Association): This is the gold standard. Request a technician certiied in Earthmover Tire Service (ETS) or Commercial Tire Service (CTS). This guarantees they are speciically trained in the hazards of large OTR (Off-The-Road) tires and multi-piece rim assembly.

• OSHA 29 CFR 1910.177 Compliance: The contractor must verify that their technicians are fully trained under OSHA's standard for servicing multi-piece and single-piece rim wheels.

2. Service Equipment & Boom Operator Credentials - Tire service trucks use their own integrated hydraulic boom cranes to lift and position heavy OTR tires. You must verify the credentials of the person operating that service truck.

3. Service provider agrees to supply or adhere to the following before work begins:

• Manufacturer Torque & Pressure Specs — Ensure they have your exact crane model's inlation data.

• On-Site Safety Protocol — Technicians must use a mandatory remote inlation cage or restraining device.

• Lock-Out/Tag-Out (LOTO) Program — Veriication that the crane will be properly blocked and de-energized.

4. Once contract is awarded, all Vendor employees visiting the base will be required to complete a DIBIDS form and provide two forms of ID which the Visitor Control Center will review the paperwork and conduct a background check.
