Attachment 2: SAMPLE Task Order
SAMPLE Task Order
Introduction:
Listed below is a sampling of textbooks and/or subscriptions (offerors must NOT actually place orders for items listed below). Offerors must provide a Sample Task Order proposal with a separate Contract Line Item Number (CLIN) for the School of Medicine (SOM), Graduate School of Nursing (GSN), and Learning Resource Center (LRC). Each CLIN must include prices that include texts and/or subscriptions in hardcopy and, if also available, electronic format.
Offeror’s proposals must describe the process necessary for USUHS staff to place an order, minimum lead times for orders and/or changes to orders, and any other pertinent and necessary information.
SOM Texts (CLIN 0001):
- 190 each “Beckmann & Ling’s Obstetrics and Gynecology” by Casenova, et al., , 8th ed., ISBN# 978- 1496353092, hard or soft cover.
- 205 each “Atlas of the Heart - Mapping Meaningful Connection and the Language of Human Experience” by Brene Brown, 1st ed., ISBN# 978-0399592553, hardcopy.
