BADGE REQUEST FOR UNCLASSIFIED CONTRACT REQUIRING PHYSICAL ACCESS

TO NAVAL UNDERSEA WARFARE CENTER DIVISION NEWPORT

(NUWCDIVNPT)

BADGE REQUEST CAN ONLY COME FROM THE NUWC POC on behalf of the contractor/company.

Requirements for a NUWC badge:

1. Unescorted access to NUWC is at least three times per week for a period of at least 90 days.

2. Unescorted physical access is required INSIDE of a NUWCDIVNPT building.

3. The OF306 (Declaration for Federal Employment) and resume MUST BE completed.

4. The OF306 and resume are submitted to the NUWC Security Office through the following:

a. Hand carry to the Security Office, Bldg. 80 1st floor;

b. Send to https://safe.apps.mil/. Put the email address:

camille.samson@navy.mil when prompted for the recipient.

5. The electronic questionnaire for investigations processing (eQIP) will be initiated AFTER the OF306, resume, and proof of citizenship are submitted.

6. Access to a computer is REQUIRED to complete eQIP.

7. The eQIP will be rejected and returned for correction if there are errors or missing information.

8. Fingerprints and verification of citizenship are required before we release eQIP. Citizenship verification requires a US Birth Certificate, US Passport, or

Naturalization Certificate.

9. A NUWC badge will be issued once a favorable result from the Advanced

Fingerprint check is received.

10. The NUWC badge WILL NOT BE ISSUED if the Advanced Fingerprint report is not favorable. The applicant must then wait for the final results of the background investigation.
